Se ha pasado de una concepción limitada únicamente a las adicciones a sustancias químicas, denominadas drogodependencias o toxicomanías, a la inclusión, en los últimos años, de otras conductas con causas y efectos similares a las drogodependencias, pero en las cuales no hay droga: son las adicciones sin sustancia. También es cierto que el concepto de adicción se ha vulgarizado y en la actualidad se utiliza para muchas conductas repetitivas, sin ninguna base científica, situación muy generalizada entre ciudadanos legos en la materia, frecuentemente sustentada por los medios de comunicación, pero que también es utilizado incluso entre profesionales. En la actualidad la comunidad científica cuenta con dos instrumentos que se elaboran a partir de los datos y aportaciones de un gran número de especialistas y que califican los posibles trastornos: la Clasificación Internacional de Enfermedades, y el Manual diagnóstico y estadístico de los trastornos mentales. Ambos instrumentos consideran que existen patrones similares entre las adicciones con y sin sustancia y que son más los elementos que les unen frente a los que le separan.
