Book excerpt: A patient who underwent a right total hip arthroplasty was seen for physical therapy treatment for four sessions from July 17th to July 19th, 2014 at an acute care hospital under the supervision of a licensed physical therapist. The patient was evaluated at the initial encounter with the Numeric Pain Rating Scale, manual muscle testing, "6 Clicks" Basic Mobility Inpatient Short Form, and functional mobility level of assistance assessment, and a plan of care was established. Inpatient goals for the patient included becoming independent in bed mobility, transfers, household ambulation with a front wheel walker, and in the performance of a home exercise program. Main interventions included patient education, therapeutic exercise, and repetitive training of functional activities such as bed mobility, transfers and gait training. The patient achieved all established inpatient goals. The patient was discharged to her home where she had 24 hour assistance available from her husband, and home health physical therapy.

Main interventions used were task specific transfer training, strength, and mobility training. The patient achieved improved functional independence, ROM, and strength. The patient was discharged to home with outpatient physical therapy.

The patient was evaluated at the initial encounter with the Well's Clinical Prediction Rule to assess for the presence of deep vein thrombosis; the Numeric Pain Rating Scale was used to assess pain; the Physical Function ICU Test--Scored was used to assess strength; the Activity Measure for Post-Acute Care mobility short form was used to assess functional mobility; the 10 Meter Walk Test was used to assess gait speed; the Lower Extremity Function Scale was used to assess independence with ADLs; a plan of care was established based on the results of these outcome measures. Main goals for the patient were to improve pain, range of motion, strength, functional mobility, gait speed, and functional independence. Main interventions used were over-ground gait training, restorative training, resistive exercises, bed mobility and transfer training, and functional training. The patient improved in pain levels, strength, range of motion, functional mobility, gait speed, and functional independence. The patient was discharged to home with a home exercise program and a plan to follow-up with home health physical therapy.

The main goals for the patient were to decrease pain severity, improve basic functional mobility, increase gait speed, and advance health related quality of life. The principal interventions used were resistance training, over-ground gait training, task-specific training, and functional training. The severity of the patient's pain improved significantly [75%], and, to a lesser degree, basic functional mobility and gait speed improved as well. The patient was discharged to an extended care facility for further physical therapy treatment.

The main goals for the patient were to improve strength and range of motion (ROM), decrease pain and improve functional mobility, including bed mobility, transfers, ambulation and stair climbing. The main interventions used were therapeutic exercise and task specific training. The patient improved strength and ROM, decreased pain, and improved independence in bed mobility, transfers, gait and stair training. The patient was discharged home with a home exercise program and follow up with outpatient physical therapy.

The overall interventions approach was progressive strengthening, task specific training, early weight bearing, and patient education. The patient's ROM, strength, balance, independence in bed mobility, transfers, and gait improved significantly.
