The review documented in this report was conducted over a four-month period by a project team of railway safety, risk management and regulatory experts. The study led to the identification of a number of railway safety system issues and the development of system improvements to address the issues. The system improvements recommended by the review will enhance the ability of the railway system to give reasonable assurance of the continuing state of railway safety.
