Book excerpt: The Global Framework for Action 2024–2030 for water, sanitation, hygiene, waste and electricity services in all health care facilities (the “Framework”) serves to guide efforts to deliver safe and sustainable water, sanitation and hygiene (WASH), health care waste management and reliable electricity in all health care facilities. The ultimate aim is to provide quality care for all. The Framework reflects a global consultative process and includes data and recommendations articulated in recent WHO/UNICEF global reports on WASH, waste and electricity in health care facilities. It also provides an operational roadmap for implementing the 2023 United Nations General Assembly (UNGA) resolution on WASH, waste and electricity in health care facilities. The target audiences for this Framework include health leaders and programme managers at the global and national levels; policymakers; WASH, waste and energy leaders and technical experts; development partners and finance institutions; and actors and experts on gender equality, disability and social inclusion and climate; and, more generally, civil society. The Framework deals with the WASH, waste and electricity elements of the WHO comprehensive approach to build safe, climate-resilient and environmentally sustainable health care facilities.

Available in PDF, EPUB and Kindle. Book excerpt: The new 2030 Agenda for Sustainable Development includes water, sanitation, and hygiene (WASH) at its core. A dedicated Sustainable Development Goal (SDG 6) declares a commitment to "ensure availability and sustainable management of water and sanitation for all." Monitoring progress toward this goal will be challenging: direct measures of water and sanitation service quality and use are either expensive or elusive. However, reliance on household surveys poses limitations and likely overstated progress during the Millennium Development Goal period. In Innovations in WASH Impact Measures: Water and Sanitation Measurement Technologies and Practices to Inform the Sustainable Development Goals, we review the landscape of proven and emerging technologies, methods, and approaches that can support and improve on the WASH indicators proposed for SDG target 6.1, "by 2030, achieve universal and equitable access to safe and affordable drinking water for all," and target 6.2, "by 2030, achieve access to adequate and equitable sanitation and hygiene for all and end open defecation, paying special attention to the needs of women and girls and those in vulnerable situations." Although some of these technologies and methods are readily available, other promising approaches require further field evaluation and cost reductions. Emergent technologies, methods, and data-sharing platforms are increasingly aligned with program impact monitoring. Improved monitoring of water and sanitation interventions may allow more cost-effective and measurable results. In many cases, technologies and methods allow more complete and impartial data in time to allow program improvements. Of the myriad monitoring and evaluation methods, each has its own advantages and limitations. Surveys, ethnographies, and direct observation give context to more continuous and objective electronic sensor data. Overall, combined methodologies can provide a more comprehensive and instructive depiction of WASH usage and help the international development community measure our progress toward reaching the SDG WASH goals.

Many countries are taking action, but more collaborative, focused and expansive effort and investments are needed. This report focuses on global and national efforts and progress to improve water, sanitation, hygiene (WASH), cleaning and health care waste management in health care facilities. It builds upon the first update of progress published in 2020. In addition, in response to demands from countries and health actors on providing a consolidated package of information and interventions on climate-resilient and sustainable infrastructure services in health care facilities, data on electricity and examples of electrification are included. It includes a summary of country progress in implementing national actions (“practical steps”) articulated in the 2019 World Health Assembly Resolution on WASH in health care facilities, drawing on data from a “country tracker” and provides insights on successful approaches and challenges from country experiences in improving and sustaining WASH in health care facilities. The main audiences for the report are national health authorities; those engaged in WASH and infection prevention and control (IPC) in health care facilities, including ministries of health, water, infrastructure, energy and financing; global and national WASH and health partners, including donors and health funders; and actors working on climate-resilient and low-carbon health care facilities and health systems.

This document also provides key information on selected health and development issues for TWG WSH member countries. The member countries covered in this report are: Brunei Darussalam Cambodia China Indonesia Japan the Lao People s Democratic Republic Malaysia Mongolia Myanmar the Philippines the Republic of Korea Singapore Thailand and Viet Nam. Previous documents prepared by WHO for the TWG WSH include extensive analyses of the drinking-water sanitation and hygiene sector as a whole based on country-level information. A key finding of this report is that the TWG WSH region has succeeded in meeting the Millennium Development Goal (MDG) target for drinking-water and sanitation which is to halve by 2015 the proportion of the population without sustainable access to safe drinking-water and basic sanitation (UN 2017). Individually nine TWG WSH countries achieved the MDG sanitation target but four countries (i.e. Cambodia Indonesia Mongolia and the Philippines) did not. All TWG WSH countries met the MDG drinking-water target except Mongolia which fell short of the target by 13 percentage points. Overall 77% of people in the TWG WSH region are using improved sanitation facilities whereas 94% are using improved drinking-water sources (UNICEF and WHO 2015). Despite an impressive effort made to provide drinking-water and sanitation infrastructure to the residents in this region about one quarter of the population still lacks access to improved sanitation and 7% lacks access to improved drinking-water. Water-related diseases including diarrhoeal diseases are significant causes of death among children under 5 years old in the region. Almost 30 000 people in the TWG WSH region especially children under 5 die each year due to water sanitation and hygiene related diseases (WHO 2014).

Book excerpt: As part of the Sustainable Development Goals introduced by the United Nations in 2015, SDG 6 aims to achieve environmentally sustainable and equitable water, sanitation, and hygiene accessibility for all. WASH-focused policies produced by health organizations use SDG 6 to guide their strategies for investments in WASH infrastructure from many different angles (i.e., capacity-building, policy and advocacy, direct financial support), but there is still a lack of transparency in the roles and responsibilities of key health organizations due to the complexity behind WASH-related problems. This research investigates the WASH strategies of four leading organizations (UNICEF, WHO, Joint Monitoring Program, World Bank) that analyzes both the apparent investment roles and associated WASH infrastructure as well as the fundamental driving values that each organization displays in their strategies. Through an inductive qualitative study design, thirty-seven publicly available strategy documents and two in-depth interviews were collected and analyzed. The results were summarized into two parts: code frequency analysis and thematic analysis. The code frequency analysis revealed variable WASH infrastructure investment preferences but similarities in capacity-building and policy and advocacy roles between UNICEF and WHO, predictable roles in WASH monitoring for JMP, and direct financial support for the World Bank. The thematic analysis demonstrated that most of the policies (~60%) included in the study showed integrated values such as equitable WASH access in tandem with the conservation of the natural environment. However, ~30% of the policy was defined by singular values of either human health, the natural environment or social equity and only a very small percentage integrated all three of these values. In order to more thoroughly achieve the integrated nature of SDG 6, the policies of these organizations should aim to become more integrated if they are to meet the WASH demands of global populations.

This document outlines the latest research on the burden of diarrhoea related to inadequate water, sanitation and hygiene (WASH). It is based on a series of articles published in the scientific literature. In bringing together current evidence on exposure to unsafe drinking-water, inadequate sanitation and hygiene, alongside the most up-to-date analysis on the health impacts of interventions, this document contributes to informed policymaking and targeting of resources. It underscores how further progress can be achieved in this unfinished global water and sanitation and health agenda.
