Breast reconstruction with prosthesis is a successful surgical treatment method after mastectomy. However, capsular contracture, a fibrous scar tissue surrounding implants with unknown definitive cause, is a frequent dilemma for plastic surgeons. The purpose of this literature review is to discuss some of the risk factors of developing capsular contracture and how acellular dermal matrices play a role in effective surgical correction. Methods: A PubMed search utilizing the online resources of the Weill Cornell Medical College Library was performed for articles that discussed capsular contracture following breast reconstruction surgery. Only full-text, English, peer-reviewed journal articles were selected for this literature review. Results: The studies showed that capsular contracture is a multifactorial phenomenon that has no one absolute cause. Inflammation, infection, hematoma, radiation, and patients' individual comorbidities all may play a role in the formation of capsular contracture. The lack of a single, definitive cause poses a difficult task for treatment. These studies concluded that surgical correction with the addition of an acellular dermal matrix offers successful outcomes. Alloplastic breast reconstructions with acellular dermal matrix developed less capsular contracture than those without. Conclusions: Breast reconstruction is considered a medical necessity as part of breast cancer treatment. These patients are an important population for the continued study of complication treatments, namely capsular contracture, to improve aesthetic outcomes and reduce future reoperation.
