Cette façon si particulière de prendre en charge la patiente et le couple m'a interpellé et m'a donné l'envie d'en savoir plus. Le but de ce travail est de montrer s'il existe un réel impact de l'accompagnement global sur l'issue de la grossesse, de l'accouchement et du post-partum. Pour se faire j'ai réalisé une étude sur deux populations de patientes : quinze patientes ayant bénéficié de l'accompagnement global et quinze patientes ayant bénéficié d'un accompagnement classique en structure hospitalière. J'ai étudié le vécu de la grossesse, de l'accouchement et du post-partum ainsi que des indicateurs périnatals tels que le taux d'extraction instrumentale, de césarienne, d'épisiotomie, . ..etc. Globalement, d'un point de vue qualitatif, le vécu de ces patientes semble être meilleur lors d'un accompagnement global. Il en est de même quant aux résultats des indicateurs périnatals. Au regard des témoignages recueillis, nous constatons que les couples sont pleinement satisfaits d'avoir vécu l'attente et la venue de leur enfant dans les conditions du suivi global proposé par les sages-femmes. Le respect de la physiologie, le respect de chacun, le respect des volontés, l'écoute, le soutien sont pour les couples autant de priorités à ne pas négliger pour vivre une maternité heureuse. Ce constat doit être pris en compte pour améliorer le suivi des femmes à l'hôpital. Une réorganisation du secteur hospitalier, la création de salles d'accouchements moins médicalisées pour les patientes à bas risque seraient dans un futur proche les clés de la réussite.

Book excerpt: Sous l'influence d'une société où la grossesse et l'accouchement sont perçus à risque, la majorité des femmes recherchent un accouchement médicalisé. Cependant cette surmédicalisation est anxiogène pour les femmes et peut être à l'origine de perturbations pouvant aller jusqu'au choc post-traumatique. De plus en plus de femmes dénoncent cette médicalisation et s'orientent vers une alternative à la prise en charge de l'accouchement. L'objectif de cette étude était de recueillir les motivations des femmes ayant bénéficié d'un accompagnement global et d'identifier le poids des influences sur cette motivation. Nous avons mené une étude qualitative auprès de 12 femmes ayant choisi cette prise en charge. L'analyse du discours a mis en évidence que la maternité hospitalière était considérée comme un lieu non sécurisant où certains actes étaient associés à de la violence obstétricale. Les femmes recherchaient prioritairement un environnement sécure, comme à la maison : un lieu chaleureux et intime, en présence d'une sage-femme avec qui elles ont construit une sécurité émotionnelle. Cette sécurité émotionnelle est basée sur une relation de confiance, sur la disponibilité de la sage-femme, sur des compétences spécifiques, et la place exceptionnelle du conjoint permettant à la femme de vivre son projet de naissance. La question de l'autonomie à retrouver est centrale pour ces femmes interrogées. Malgré l'influence forte de la société, le choix de l'accompagnement global est motivé par le désir d'un accomplissement personnel de l'accouchement naturel.
