Il existe près de 7000 maladies rares connues à ce jour, soit plus de 3 millions de personnes en France. 80% des maladies rares sont d'origine génétique et 95% d'entre elles ne bénéficient pas de traitement curatif. Ces maladies sont souvent graves, chroniques, évolutives, invalidantes, et peuvent engager le pronostic vital. Du fait de leur rareté, leur hétérogénéité, leur étrangeté, la faible prévalence de malades, et parce que les personnes atteintes sont bien trop souvent dans une situation d'errance diagnostique et d'isolement, les maladies rares nécessitent la mise en place d'une prise en charge adaptée et de répondre aux besoins spécifiques d'accompagnement des patients. Grâce à l'offensive des associations de malades et la création de l'Alliance maladies rares, les maladies rares représentent aujourd'hui une préoccupation de santé et bénéficient du soutien des pouvoirs publics, notamment grâce à la mise en œuvre d'un Plan National Maladies Rares pour améliorer la prise en charge des patients. En ville, le pharmacien à un rôle à part entière à jouer dans le parcours de soins coordonné des personnes porteuses de maladies rares, en tant qu'acteur de santé de proximité. Dans cette thèse, nous avons analysé comment et pourquoi le pharmacien d'officine pouvait être un pilier dans l'accompagnement de ce type de patients. Pour cela, nous avons réalisé une enquête pilote en Nouvelle-Aquitaine entre février et juin 2021, à l'aide de questionnaires établis à l'attention de 3 profils cibles : des étudiants en pharmacie, des pharmaciens d'officine, ainsi que des patients porteurs de maladies rares. Le but était de comprendre les besoins et les attentes de chacun, respectivement au cours de leurs études, de leur pratique professionnelle et dans leur prise en charge, puis de cerner leurs représentations autour des maladies rares, et des uns vis-à-vis des autres : afin de progresser dans la connaissance de ces maladies, ainsi que dans l'espoir de proposer des solutions et des outils nécessaires à l'accompagnement attentif des personnes porteuses de maladies rares pour le pharmacien exerçant en officine.

Souvent isolés, les patients et leur famille, à force de combats, ont réussi à faire de celles-ci un enjeu majeur de santé publique. Même si de nos jours aucun traitement n'existe, la compréhension de la physiologie de ces pathologies a permis d'ouvrir des pistes thérapeutiques. En axant ce travail sur les pathologies rares mitochondriales, nous avons cherché 1) à définir les différents approches thérapeutiques en cours aujourd'hui; 2) à évaluer dans une pathologie mitochondriale donnée, la NOHL explorée au CHU de Bordeaux, quelle était la prise en charge proposée 3) à évaluer la place de l'officine dans cette prise en charge. A l'issu de ce travail il transparait que les traitements actuellement proposés sont réservés et ont l'exclusivité hospitalière. Il sera donc difficile de se voir un jour proposer ces traitements dans nos officines. Néanmoins, au regard de ces maladies rares, le pharmacien d'officine restera un acteur de santé indispensable dans la relation patient-médicament, - Il devra être vigilant sur la délivrance de médicaments pouvant aggraver l'état de santé de ces patients. - Il devra rester à l'écoute et accompagner le patient et sa famille dans leur quotidien. - Il devra être le relai entre les différentes structures de prise en charge que ce soit les associations de malades, les centres de références et de compétences ou le CHU.

Motivated by an impulse that, as he himself says, ‘was civic rather than political’, nothing seemed more important to him than to understand the Algerian situation and provide the elements that would enable others to come to an informed judgement about it. In extremely tough conditions and along with a small group of students, Bourdieu undertook a series of studies across an Algeria that was tightly patrolled by the army, leading him to discover the shocking reality of the resettlement camps and to analyse the mechanisms of destruction of Algerian society of which they were emblematic. To achieve the objectives he had set himself, Bourdieu had to carry out a genuine intellectual conversion, acquiring an ethnographic understanding of Algerian society, learning sociological analysis at a breakneck pace and inventing new instruments - both theoretical and empirical - that would enable him to understand the relations of domination specific to colonialism. These new tools also enabled him to analyse the nature of the crisis that the war had both produced and manifested.
