Book excerpt: Neonatal seizures represent one of the most frequent neurological events in newborn infants, often reflecting a variety of different pre-, peri-, or postnatal disorders of the central nervous system (CNS). They are also a common manifestation of metabolic abnormality in newborn period and often represent the first sign of neurological dysfunction in neonates. They may be symptomatic or cryptogenic, herald subsequent epilepsy, can be associated with potential morbidity and mortality, and may be used as a factor in considering long-term prognosis. Despite the enormous clinical significance of these events, many aspects of their management are not well supported with evidence- based recommendations. These guidelines are intended to be of use for neonatologists, paediatric neurologists, paediatricians, general practitioners, nurse practitioners, nurses and other health professionals who may be in contact with infants experiencing seizures within the first 28 days of life (age up to 44 weeks postconception). The guidelines are framed so as to be applied by health care providers practicing in a wide range of health care facilities, from those with limited resources to tertiary care centers.

In the last decade, advances in auditory science and technology have not only greatly expanded the utility of electric stimulation to other parts of the auditory nervous system in addition to the cochlea, but have also demonstrated drastic changes in the brain in responses to electric stimulation, including changes in language development and music perception. Volume 20 of SHAR focused on basic science and technology underlying the cochlear implant. However, due to the newness of the ideas and technology, the volume did not cover any emerging applications such as bilateral cochlear implants, combined acoustic-electric stimulation, and other types of auditory prostheses, nor did it review brain plasticity in responses to electric stimulation and its perceptual and language consequences. This proposed volume takes off from Volume 20, and expands the examination of implants into new and highly exciting areas. This edited book starts with an overview and introduction by Dr. Fan-Gang Zeng. Chapters 2-9 cover technological development and the advances in treating the full spectrum of ear disorders in the last ten years. " Thus, in recent years, the sad lament of the patient with primary progressive MS (PPMS). This variant, often in the guise of a chronic progressive myelopathy or, less commonly, progressive cerebellar or bulbar dysfunction, usually responds poorly to corticosteroids and rarely seems to benefit to a significant degree from intensive immunosuppressive treatments. In recent years, most randomized clin ical trials have excluded PPMS patients on two counts. Clinical worsening devel ops slowly in PPMS and may not be recognized during the course of a 2-or 3-year trial even in untreated control patients. This factor alone adds to the potential for a type 2 error or, at the very least, inflates the sample size and duration of the trial. In addition, there is mounting evidence that progressive axonal degeneration and neuronal loss (rather than active, recurrent inflammation) may be important components of the pathology in this form of the disease. Although contemporary trials are evaluating whether PPMS patients may benefit from treatment with the ~-interferons and glatiramer acetate, preliminary, uncontrolled clinical experi ence suggests that the results may not be dramatic.

We have come a long way from the Latin term "hallucinari", meaning to talk nonsense, to be absent-minded, to the modern con cept of "hallucinations". While the Latin word was descriptive of what may be due to hallucinations, the modern concept defines hal lucinations as subjective experiences that are consequences of men tal processes, sometimes fulfilling a purpose in the individual's mental life.
