The special interest and importance, in a surgical sense, Of the great European War lies not so much in the fact that examples of every form of gross lesion of organs and limbs have been seen, for if we read the Older writers we find little in the modems that is new in this respect, but is to be found in the enormous mass of clinical material which has been presented to us and in the production of evidence sufficient to eliminate sources of error in determining important conclusions. For the first time also in any campaign the labours of the surgeon and the physician have had the aid of the bacteriologist, the pathologist, the physiologist, and indeed of every form of scientific assistance, in the solution of their respective problems. The clinician entered upon the great war armed with all the resources which the advances of fifty years had made available. If the surgical problems of modern war can be said not to differ sensibly from the campaigns of the past, the form in which they have been presented is certainly as different as are the methods of their solution. The achievements in the field of discovery of the chemist, the physicist, and the biologist have given the military surgeon an advantage in diagnosis and treatment which was denied to his predecessors, and we are able to measure the effects of these advantages when we come to appraise the results which have been attained. But although we may admit the general truth of these statements it would be wrong to assume that modern scientific knowledge was, on the outbreak of the war. Large numbers of patients all over the country are admitted to emergency rooms because of abdominal trauma of varying degrees of severity. All too often the correct diagnosis is suspected belatedly or not at all, so that proper treatment is not initiated in sufficient time to be lifesaving. Not infre quently, the injured patient is examined by an intern or an insufficiently experienced resident physician. Even in instances where more senior internists and surgeons are available, detailed knowledge about the necessary methodology to establish the correct diagnosis and institute the appropriate treatment is lacking. This monograph, representing the felicitous collaboration of a surgeon and a radiolo gist together with several other contributors, is timely and important. The authors (and their contributors) have approached their subject with a wealth of clinical experi ence obtained in several very active acute-care municipal hospitals in the largest city in this country. The continued advance of our knowledge of the acute conditions which may arise in the abdomen has necessitated an increase in size of the volume, whilst it has appeared desirable to add the accounts of many cases and illustrations with the view of more thoroughly elucidating the subject. So many acute abdominal diseases have proved curable by operation, when it is performed early, that I hope my readers will not consider the subject unduly magnified considering its importance in modern practice. The treatment of the acute abdomen (including that of lesions the result of traumatism) demands an acquaintance with the surgical part of the subject which cannot always be obtained in text-books on surgery. In addition, it may be added that in order to be of general application the methods described should be as simple as possible, so that no practitioner need fear to make an attempt to save a threatened life because he has not some particular clamp, bobbin or brand of catgut in his possession. Silk is still recommended, because it is strong and can be trusted. Moreover, it is easily sterilised by boiling, even in a cottage. The principal additions to the present volume, which is divided into sections, are those on the planning of abdominal incisions and injuries to the abdomen, external and internal, including most of the subject-matter of the Oration given before the Medical Society of London in 1910. Still, I confess to surprise at the interest that has been evoked, and have been almost startled at the really large number of isolated facts, bearing on the subjects discussed, that have been brought to ones notice. Thus Dr. Spilsbury, who tells me that he has not always found the coeliac axis degenerate when there has been pretty widespread arterio-sclerosis, says that nevertheless in such cases there is often marked hypertrophy of the muscular coat of the vessel. The relevancy of this point to the suggestion of spasm and excessive adrenalism is obvious. In respect of the second paper, during discussion both Sir John Tweedy and Dr. F. J. Smith narrated some cases that were very a propos, and I have heard since of many others. Mr. Roland Burrows, himself a lawyer, raised the interesting point whether indeed the neurasthenia of twentieth-century males may not be a consequence of "repression," and whether the emotional displays of our eighteenth-century heroes and statesmen - who lived in shocking times - may not have been physiologically justified. Such suggestions give an even wider significance to the Freudian doctrines. The comments of the anaesthetists are very interesting. The declaration by not a few that "surgical shock" is very often a matter of overdeep anaesthesia seems based on sound clinical observation, and should be correlated with Verwom's masterly elucidation of the phenomena of narcosis.
