Each succeeding chapter, describes the problems in medicine, followed by a review in chronological sequence of why and how computers were applied to try to meet these problems. This approach generally led to defining the objectives for applications of medical informatics. At the end of each chapter, the author summarizes his personal views and interpretations of the chapter contents. Although the concurrent evolution of medical informatics in Canada, Europe, and Japan certainly influenced workers in the United States, the scope of this historical review is limited to the development of medical informatics within the United States. Furthermore, this review is limited to electronic digital computers; it excludes mechanical, analog, and hybrid computers.

Book excerpt: As research on expert systems has moved well into its second decade, it has become popular to cite the limitations of the phenomenologic or associational approach to knowledge representation that was typical of first generation systems. For example, the Internist-1 knowledge base represents explicitly over 600 diseases, encoding associated disease manifestations (signs, symptoms, physical findings, and lab abnormalities) but failing to deal with the reasons that those findings may be present in the disease [Miller, R. A. 82]. In recent years Pople has sought to add detailed causal models to the knowledge base in a revised version of the program known as CADUCEUS [Pople 82]. Similarly, a typical production rule in the MYCIN system states inferences that may be drawn when specific conditions are found to be true [Buchanan 84], but the underlying explanations for such relationships are not encoded. Clancey has argued that MYCIN needs such "supporting knowledge" represented, especially if its knowledge base is to be used for teaching purposes [Clancey 83]. By the late 1970s, artificial intelligence researchers were beginning to experiment with reasoning systems that used detailed mechanistic or causal niodels of the object being analyzed. Among the best early examples were a program to teach students how to analyze electronic circuits [Brown 82] and a system for diagnosing problems with mechanical devices [Rieger 76].

Enormous strides have been made to improve the quality of life of millions of people through the application of the scientific discoveries made during this and past centuries. The 20th century will be remembered as much for the mass exploitation of scientific discovery as for the discoveries themselves. The technological age has meant that the human being is able to contemplate activities which "defy" nature. For example, some of the work involved in the preparation of these proceedings has been done whilst travelling at over 500 miles per hour seven miles above the surface of the earth. It is not difficult to conjecture about the effect that this relatively recent technology has had upon a number of "systems". Air transportation has provided a number of benefits including such disparate examples such as enabling holidays, famine relief and the cross fertilisation of cultural practices from other lands. Equally, there have been undesirable effects such as enabling the means of mass destruction, interference in other cultures and the speedy transportation of disease. Moreover, the physical presence of the aeroplane itself represents the consumption of fossil fuels, a source of pollution and a change in the way think about life. The view expressed here is of course the view of an inhabitant of the "western world".
