Book excerpt: Over 3 million U.S. military personnel were sent to Southeast Asia to fight in the Vietnam War. Since the end of the Vietnam War, veterans have reported numerous health effects. Herbicides used in Vietnam, in particular Agent Orange have been associated with a variety of cancers and other long term health problems from Parkinson's disease and type 2 diabetes to heart disease. Prior to 1997 laws safeguarded all service men and women deployed to Vietnam including members of the Blue Navy. Since then, the Department of Veteran Affairs (VA) has established that Vietnam veterans are automatically eligible for disability benefits should they develop any disease associated with Agent Orange exposure, however, veterans who served on deep sea vessels in Vietnam are not included. These "Blue Water Navy" veterans must prove they were exposed to Agent Orange before they can claim benefits. At the request of the VA, the Institute of Medicine (IOM) examined whether Blue Water Navy veterans had similar exposures to Agent Orange as other Vietnam veterans. Blue Water Navy Vietnam Veterans and Agent Orange Exposure comprehensively examines whether Vietnam veterans in the Blue Water Navy experienced exposures to herbicides and their contaminants by reviewing historical reports, relevant legislation, key personnel insights, and chemical analysis to resolve current debate on this issue.

Book excerpt: Delinquency in a Birth Cohort, published in 1972, was the first criminologi cal birth cohort study in the United States. Nils Christie, in Unge norske lovorertredere, had done the first such study as his dissertation at the University of Oslo in 1960. Professor Thorsten Sellin was the inspiration for the U.S. study. He could read Norwegian, and I could a little because I studied at the University of Oslo in my graduate years. Our interest in pursuing a birth cohort study in the United States was fostered by the encouragement of Saleem Shah who awarded us a grant from the National Institute of Mental Health to begin our birth cohort studies at the University of Pennsylvania by investigating the delinquency of the 1945 cohort. We studied this group of 9,945 boys extensively through official criminal history and school records of their juvenile years. Subsequently, we followed up the cohort as adults using both adult arrest histories and an interview of a sample of the cohort. Our follow-up study was published as From Boy to Man, From Delinquen cy to Crime in 1987.

Having spent 2 years as Director of the Law and Social Science Program at the National Science Foundation, I was well aware of the rich diversity of research that could fit within that topic. I also knew that excellent research on youth and justice was coming from different communities of researchers who often were isolated from each other in their respective disciplines as psychologists, sociologists, criminologists, or policy analysts. I saw this project as an opportunity to break down some of this isolation by introducing these researchers-and their work-to each other and to the broader community of social scientists interested in law and justice. There was another gap, or set of gaps, to be bridged as well. The juvenile justice system and the criminal justice system differ in significant ways, and the civil justice system, which is a major venue for issues of youth and justice, is yet another separate world. Few researchers are likely to know the whole picture. For example, a focus on juvenile justice often ignores the extent to which civil justice proceedings shape the lives of young people through divorce, custody, adoption, family preservation policies, and other actions (and vice versa).

Book excerpt: Since their first application in 1982, Totally Implantable Venous Access Devices (TIVADs) have become increasingly important in the clinical practice, as more intensive chemotherapy and parenteral treatments have come into use. At this time, there is objective evidence that TIVADs are a safe, effective strategy for long-term venous access; they play a significant role throughout the management of the oncology patient, as they are needed in the initial phases for active treatments as well as in the last stages for palliative measures, making possible repeated administration of chemotherapeutic vesicant agents, nutrients, antibiotics, analgesics, and blood products. According to a number of prospective studies, use of TIVADs is associated with a significant complication rate (10% to 25% of all patients). Evidence-based data support that most complications are directly related to inappropriate technique of placement and/or nursing care, sometimes leading to TIVAD loss, significant morbidity, increased duration of hospitalization, and additional medical cost. A group of world-renowned experts - both in the clinical and research fields – contributed to this volume, whose aim is to provide clinicians, nurses and medical students with a multidisciplinary, full update on these devices, as long term central venous access can no be longer considered a routine matter, and serious complications can be maintained at a very low level only if strict adherence to a well-defined protocol of surgical technique and of catheter care is maintained.
