Book excerpt: Explosions and blast waves can cause human injuries due to both the interaction of the blast waves with the human tissues and the generated and propelled fragments that strike the body. Several models for the prediction of these injurious effects have been reviewed in this report. They have been selected from those most widely established and more suitable for implementation in finite element codes for risk assessment in large-scale numerical simulations. The models examined have been presented according to the established three categories of primary, secondary and tertiary injury mechanisms. They are probabilistic and their probit (or logistic) functions are presented and explained. Appropriate pressure-impulse and mass-velocity diagrams are drawn for blast waves and fragment injuries, respectively. Comparisons of injury criteria predictions are made, and some merits or shortcomings of the models are indicated. A workable model of munition fragmentation is also included. Thus the report provides the means for an efficient assessment of human injury risk in case of explosion events, which can effectively contribute to refining methods of protection in security and defence.

Before the meeting, a conference planning committee consulted on the literature review and research questions and served as a peer review panel for submitted abstracts. Five leading scientists and clinicians in related fields were invited to serve on an expert panel, lead working groups, and develop overall recommendations. These conference proceedings summarize the meeting and present key findings.

Military personnel and civilians often experience blast loading to the lower extremity due to its direct contact with the ground and floor of vehicles. The pressure and axial loading from these incidents often lead to detrimental injuries. These injuries can be due to a number of mechanisms terming them primary, secondary, tertiary, or quaternary blast injuries. Of these injuries, this study will focus on primary and tertiary injuries, specifically bone fractures, compartment syndrome, and soft tissue disruption which often result from blast loading due to these mechanisms. However, the pressure and load levels causing these injuries are unknown. Currently, the methodologies, which study the injury criteria and design of blast mitigating structures, are limited. The main limitations are the lower rates of testing (automobile), specimen limitation (cadavers, surrogates, etc.), cost, and testing repeatability. Consequently, the goal of this dissertation is to develop a realistic computational model which can be used to improve the injury criteria, personal protective equipment (PPE), and vehicular structure in a cost effective and timely manner. Three Aims were thus pursued. For Specific Aim 1, a standing lower extremity was developed, verified, and simulated with several open-air blast loading conditions. Specific Aim 2 focused on validating the lower extremity model using experimental drop tower test results. In the drop tower simulation, the lower extremity model was successfully converted into a seated posture model and setup with similar loading and boundary conditions as the experiment. Specific Aim 3 involved incorporating a boot into the standing lower extremity model and evaluating its ability to mitigate pressure waves. In summary, Specific Aims 1 and 2 developed, verified, and validated a realistic human lower extremity model for the use in blast simulations. Specific Aim 3 further confirmed the models use in developing PPE.

Book excerpt: Advances in trauma care have accelerated over the past decade, spurred by the significant burden of injury from the wars in Afghanistan and Iraq. Between 2005 and 2013, the case fatality rate for United States service members injured in Afghanistan decreased by nearly 50 percent, despite an increase in the severity of injury among U.S. troops during the same period of time. But as the war in Afghanistan ends, knowledge and advances in trauma care developed by the Department of Defense (DoD) over the past decade from experiences in Afghanistan and Iraq may be lost. This would have implications for the quality of trauma care both within the DoD and in the civilian setting, where adoption of military advances in trauma care has become increasingly common and necessary to improve the response to multiple civilian casualty events. Intentional steps to codify and harvest the lessons learned within the military's trauma system are needed to ensure a ready military medical force for future combat and to prevent death from survivable injuries in both military and civilian systems. This will require partnership across military and civilian sectors and a sustained commitment from trauma system leaders at all levels to assure that the necessary knowledge and tools are not lost. A National Trauma Care System defines the components of a learning health system necessary to enable continued improvement in trauma care in both the civilian and the military sectors. This report provides recommendations to ensure that lessons learned over the past decade from the military's experiences in Afghanistan and Iraq are sustained and built upon for future combat operations and translated into the U.S. civilian system.

That many returning veterans have TBI will likely mean long-term challenges for them and their family members. Further, many veterans will have undiagnosed brain injury because not all TBIs have immediately recognized effects or are easily diagnosed with neuroimaging techniques. In an effort to detail the long term consequences of TBI, the committee read and evaluated some 1,900 studies that made up its literature base, and it developed criteria for inclusion of studies to inform its findings. It is clear that brain injury, whether penetrating or closed, has serious consequences. The committee sought to detail those consequences as clearly as possible and to provide a scientific framework to assist veterans as they return home.
