Book excerpt: Hypertension is one of the commonest diseases with an estimated worldwide prevalence of 1 billion. Data from the 3rd National Health and Nutritional Assessment Survey reveals that in US, one-third of people were unaware of this problem and another one third had blood pressure control below established goals (JNC, 1997). To add to this is the observation in the 7th Joint National Committee on Prevention, detection, Evaluation, and Treatment of High Blood Pressure report that each increment of 20 mmHg in systolic or 10 mmHg diastolic pressure doubles the risk of cardiovascular disease (JNC, 2003). There are no similar data in children where the age, gender and height need to be taken into account while interpreting blood pressure values.

In children and adolescents, the normal range of blood pressure (BP) is determined by body size and age. BP standards that are based on gender, age, and height provide a more precise classification of BP according to body size. Studies investigating blood pressure levels in children have revealed wide variation in distribution of systolic and diastolic blood pressure as well as in the timing of spurt in mean blood pressure levels. Current guidelines define childhood hypertension as persistence of elevation of blood pressure above 95th percentile for age, gender and height. Our aim of the present study is to study the blood pressure levels in apparently healthy school children, to determine the prevalence and correlation of high blood pressure levels among children with different nutritional status and to evaluate the strength of the relationship between blood pressure and various anthropometric indicators.

Book excerpt: "Background:Elevated casual hypertension in children can be predicted by a number of factors, including excessive BMI and poor lifestyle habits. If maintained into adolescence, hypertension can lead to increased left ventricular mass index (LVMI) and carotid intima media thickness (cIMT), and eventually cardiovascular events. However, the association of 24-hours ambulatory blood pressure (BP) on these outcomes has not been thoroughly investigated. Moreover, it is unclear if the risk factors for hypertension change throughout childhood.Objectives:1. Determine which characteristics are associated with elevated blood pressure (BP) inchildhood, and whether the same characteristics similarly impact BP throughout childhood.2. Evaluate the extent to which elevated BP is associated with LVMI and cIMT.Methods:Two observational cohort studies:1. Prospective longitudinal cohort at two Quebec hospitals: visit 1: n=631, visit 2: n=564. Participants underwent a study visit, consisting of measurements of anthropometry, body composition, accelerometer data, fitness test, oral glucose tolerance test, and blood samples, as well as comprehensive lifestyle questionnaires for both patient and parents. Associations between variables and casual BP were measured to determine predictors of elevated BP.2. Prospective cross-sectional cohort at a Montreal hospital: 134 patients were recruited in this ongoing study. 24-hour ambulatory blood pressure monitoring (ABPM) and echocardiograms were performed to ascertain BP, LVMI, and cIMT. Associations between ABPM-derived BP and LVMI and cIMT were measured.Results:Longitudinal cohort: n=631, age=9.6 years at visit 1; n=564, age=11.7 years at visit 2; 40% of participants were overweight or obese at each visit. Adiposity and hereditary factors were associated with systolic and diastolic BP. BMIz was a significant predictor of future BP levels. Cross-sectional cohort: n=134, age=17.2 years; 36% were overweight or obese at the study visit. BMIz and lean body mass were associated with LVMI, while only BP was associated with cIMT. Other factors were independently associated with both measures, but were not significant when the models were adjusted for additional variables.Conclusions:Hypertension is associated with several measurable variables, although children and adolescents that present with these risk factors often go unscreened and untreated. The risk factors themselves and the associated ambulatory hypertension can lead to significant end-organ damage in adolescents. Research should focus on developing and testing better measurement indices, while policy and protocol changes should make BP screening and tracking a more widely implemented practice." --

Book excerpt: The purpose of this systematic evidence review is for the U.S. Preventive Services Task Force (USPSTF) to update its recommendation on screening for high blood pressure in children and adolescents to prevent cardiovascular disease. In 2003, the USPSTF found poor evidence that routine blood pressure measurement accurately identifies children and adolescents at increased risk for cardiovascular disease, and poor evidence to determine whether treatment of elevated blood pressure in children or adolescents decreases the incidence of cardiovascular disease. As a result, the USPSTF could not determine the balance of benefits and harms of routine screening for high blood pressure in children and adolescents, which resulted in an I recommendation. Recent data from the National Health and Nutrition Examination Survey suggest that mean blood pressure levels are rising steadily in children, as is the prevalence of childhood hypertension. This may be due to the increase in the prevalence of obesity and overweight among children, which is highly correlated with high blood pressure. Screening of asymptomatic children has the potential to detect hypertension at earlier stages, so that interventions can be initiated which, if effective, could reduce the adverse health effects of childhood hypertension in both childhood and adulthood, including cardiovascular disease and end-organ damage. This report summarizes recent and older evidence on screening and diagnostic accuracy of screening tests for high blood pressure in children, the effectiveness and harms of treatment for screen-detected, primary childhood hypertension, and the tracking of hypertension from childhood to adulthood. Using the methods of the USPSTF and with the input of members of the USPSTF, we developed an analytic framework and key questions to guide our literature search and review. Key Questions include: 1. Is screening for hypertension in children/adolescents effective in delaying the onset of or reducing adverse health outcomes related to hypertension? 2. What is the diagnostic accuracy of screening tests for elevated blood pressure in children/adolescents? 3. What is the association between hypertension in children/adolescents and hypertension and other intermediate outcomes in adults? 4. What are the adverse effects of screening for hypertension in children/adolescents, including labeling and anxiety? 5. What is the effectiveness of drug, nondrug, and combination interventions for treating primary hypertension in children/adolescents? 6. What is the effectiveness of drug, nondrug, and combination interventions initiated for the treatment of primary hypertension in children/adolescents for reducing blood pressure and other intermediate outcomes in adults? 7. What is the effectiveness of drug, nondrug, and combination interventions initiated for the treatment of primary hypertension in children/adolescents for reducing adverse health outcomes in adults related to primary hypertension? 8. What are the adverse effects of drug, nondrug, and combination interventions for treating primary hypertension in children/adolescents?
