In a nation that has traditionally been uncomfortable addressing mental illness, what factors have allowed for the rising medicalization of depression and suicide? Investigating these profound changes from historical, clinical, and sociolegal perspectives, Depression in Japan explores how depression has become a national disease and entered the Japanese lexicon, how psychiatry has responded to the nation's ailing social order, and how, in a remarkable transformation, psychiatry has overcome the longstanding resistance to its intrusion in Japanese life. Questioning claims made by Japanese psychiatrists that depression hardly existed in premodern Japan, Junko Kitanaka shows that Japanese medicine did indeed have a language for talking about depression which was conceived of as an illness where psychological suffering was intimately connected to physiological and social distress. The author looks at how Japanese psychiatrists now use the discourse of depression to persuade patients that they are victims of biological and social forces beyond their control; analyzes how this language has been adopted in legal discourse surrounding "overwork suicide"; and considers how, in contrast to the West, this language curiously emphasizes the suffering of men rather than women. Examining patients' narratives, Kitanaka demonstrates how psychiatry constructs a gendering of depression, one that is closely tied to local politics and questions of legitimate social suffering. Drawing upon extensive research in psychiatric institutions in Tokyo and the surrounding region, Depression in Japan uncovers the emergence of psychiatry as a force for social transformation in Japan.

Understanding the limitations of a dry symptomatological approach in helping clinicians grasp the intricacies of the experience, presentation, and course of mental illness, the NIMH Group on Culture and Diagnosis proposed to appraise, in close collaboration with the patient, the cultural framework of the patient's identity, illness experience, contextual factors, and clinician-patient relationship, and to narrate this along the lines of five major domains. By articulating the patient's experience and the standard symptomatological description of a case, the clinician may be better able to arrive at a more useful understanding of the case for clinical care purposes. Furthermore, attending to the context of the illness and the person of the patient may additionally enhance understanding of the case and enrich the database from which effective treatment can be planned. This reader is a rich collection of chapters relevant to the DSM-IV Cultural Formulation that covers the Cultural Formulation's historical and conceptual background, development, and characteristics. Few persons would care about such inequalities if the poor, powerless, and despised were as happy and fulfilled as the wealthy, powerful, and admired. Social research often springs from humanistic empathy and concern as much as from scholarly and scientific curiosity. An economist might observe that black Americans are disproportionately poor, and investigate racial differences in education, employment, and occupation that account for disproportionate poverty. A table comparing additional income blacks and whites can expect for each additional year of education is thus as interesting in its own right as any dinosaur bone or photo of Saturn. However, something more than curiosity underscores our interest in the table. Racial differences in status and income are a problem in the human sense. Inequality in misery makes social and economic inequality personally meaningful. There are two ways social scientists avoid advocacy in addressing issues of social stratification. The first way is to resist projecting personal beliefs, values, and responses as much as possible, while recognizing that the attempt is never fully successful. The second way is by giving the values of the subjects an expression in the research design. Typically, this takes the form of opinion or attitude surveys. Researchers ask respondents to rate the seriousness of crimes, the appropriateness of a punishment for a crime, the prestige of occupations, the fair pay for a job, or the largest amount of money a family can earn and not be poor, and so on. The aggregate judgments, and variations in judgments, represent the values of the subjects and not those of the researcher. They are objective facts with causes and consequences of interest in their own right.
