Book excerpt: Social isolation and loneliness are serious yet underappreciated public health risks that affect a significant portion of the older adult population. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Book excerpt: Objectives: Anxiety disorders (AD) and depression are often associated with impaired social interactions. The valence of social interactions can moderate physiological parameters like heart rate variability(HRV). Therefore, the present study compares adolescence with AD or depression and healthy ones regarding the perceived quality of social interactions. Furthermore, we investigate the interaction between perceived quality of social interactions, depression ,AD and HRV. Methods: Mental disorders were assessed using a standardized interview (DIA-X/CIDI) in a community-based sample subjects aged 14 to 21 years old from Dresden (Germany). The social interaction (Scale 0-100%) and HRV data were collected via Ecologic Momentary Assessment (EMA) eight times per day over the course of four consecutive days (N =1120). Weighted regression analyses adjusting for sex and age were used to examine associations between the perceived quality of social interaction and past month AD or depression. A multilevel regression was calculated for the interaction quality of social interaction*AD*depression*HRV. Results: Linear regressions revealed a significant lower quality of social interaction for participants with a major depression (b=-4.42 ; 95%CI [-8.15 -.0.67]; p=0.020) and panic disorder (b=-5.51 ; 95%CI [-10.03 -0.99]; p=0.017) compared to individuals without these diagnoses. Significant differences were also observed for participants with specific phobia (b=2.94 ; 95%CI [0.26 -5.63]; p=0.032). There were no significant differences for agoraphobia, dysthymia or social phobia, or GAS. Multilevel regression showed no significant interaction effect.Conclusion: Quality of social interactions seems to play an important role in some affective disorders, nevertheless there were no impact on physiological measures.
