Book excerpt: The root canal walls of 25 deciduous molar teeth with exposed and necrotic pulps were examined using ths scanning electron microscope. Immediately after extraction, all teeth were fixedin Karmosky's solution. The corob=nal portion of the tooth were sectioned at about 2mm above the enamel cemental junction. The mesial and distal roots were separated and either split in the mesio-distal or bucco-lingual direction. All specimens were prepared for SEM. Observations showed that all roots were infected with organisms consisting of cocci and short rods. However the distribution of organisms is not uniform throughout the canals. Bacterial invasion is most in the coronal region and reduces towards the apical region. Accompanying the bacterial invasion is root canal walls deterioration. The odontoblastic processes are the first to deteriorate followed by the predentine layer.

Book excerpt: The purpose of this study was to evaluate by light and scanning electron microscopy the effects of placing tetracycline-impregnated fibers in periodontal pockets for a period of ten days, with and without root planing therapy, on the root-associated microbial flora and surface characteristics of the root. Six patients were used in this study who met the following inclusion criteria: an all inclusive negative medical history; no history of antibiotic therapy or a dental prophylaxis within the previous three months; and patients must present with four hopeless, single rooted teeth, each sparated by one tooth or an edentulous area, with pocket depths greater than 4 mm circumferentially. The teeth used in this investigation were randomly assigned to one of the four following treatment groups: non-treated controls (C), scaling and root planing only (RP), tetracycline-impregnated fiber only (F), and scaling and root planing with tetracycline-impregnated fiber (RP/F). Ten days following placement of the fiber, the study teeth were extracted and prepared for examination by scanning electron microscopy (SEM). The root surface and root-associated dental plaque was evaluated for differences within and between treatment groups. In addition, one-half of the root from each of those specimens treated with tetracycline fibers, and four control teeth, were examined by fluorescent light microscopy for tetracycline adsorption and to determine level of penetration into root structure. The SEM observations from this study indicate that, when compared to control group specimens, placement of tetracycline-impregnated fibers into periodontal pockets for 10 days, either alone or in combination with root planing, resulted in a reduced bacterial population. Further, the subgingival bacterial population in these treatment group specimens consisted of long rods, cocci and filamentous organisms which generally exhibited morphologies consistent with non-viable organisms. In fact, only one recognizable bacterial colony was observed in the root planed and fiber treated specimen group and it also appeared non-viable. In contrast, those specimens treated by root planing alone, exhibited areas of maturing plaque with long and short rods, cocci and filamentous morphotypes present. Specimens in both treatment groups involving use of the fiber exhibited a polymorphonuclear leukocyte (PMN) infiltrate in the fibrous connective tissue attachment. Demineralized root surfaces were infrequently noted although resportion lacunae were noted in the "plaque free zone". The root surfaces examined by fluorescent microscopy showed only a superficial penetration of tetracycline with no penetration of dentinal tubules even in areas of demineralized root structure. The use of tetracycline-impregnated fiber alone or in combination with root planing apears to be superior to scaling alone with respect to depression of subgingival microbial populations and probable viability of the observed plaque deposits. However, the use of tetracycline fiber in conjunction with root planing is clearly superior due to the combined root debridement function and suppression of the subgingival microbial population.
