Book excerpt: Excerpt from A Report on Hospital Gangrene, Erysipelas and Pyaemia, 1863: As Observed in the Departments of the Ohio and the Cumberland, With Cases Appended Sir - The investigations which have been conducted in this city in relation to hospital gangrene, erysipelas, ichorrhaemia, thrombus, metastatic abcess (pyaemia, ) diphtheria, and gangrenous scarlatina, have had for their objects to ascertain: First, the nature of the causes operating to the production of the diseases; Secondly, the nature of the process set in motion; Thirdly, the prophylactic and curative agents. The affections just named are, in some of their aspects, so intimately united on the points touched in these investigations that it is impossible to disunite them for the purpose of description. The opportunities presented in the Military Hospitals of this city for the study of some of these diseases have been great, especially so in regard to hospital gangrene. For the study of diphtheria and scarlatina, except in connection with wounds, the occasions have not been numerous in these hospitals, the cases having been isolated and few. Indeed more cases have occurred in private than in public practice. Enough, however, have been observed to justify the conclusions expressed in the following pages. The report of the cases of scarlatina and diphtheria will form the subject of a separate paper. Hospital gangrene, erysipelas, gangrenous diphtheria, and scarlatina, viewed as local diseases, present, on careful study, many points of resemblance; nor are they unlike in the constitutional states wedded to them. Surprisingly, severe zoonoses and sapronoses still appear that are either entirely new (e.g., SARS), newly recognized (Lyme borreliosis), resurging (West Nile fever in Europe), increasing in incidence (campylobacterosis), spatially expanding (West Nile fever in the Americas), with a changing range of hosts and/or vectors, with changing clinical manifestations or acquiring antibiotic resistance. The collective term for those diseases is (re)emerging infections, and most of them represent zoonoses and sapronoses (the rest are anthroponoses). The number of known zoonotic and sapronotic pathogens of humans is continually growing − over 800 today. In the introductory part, short characteristics are given of infectious and epidemic process, including the role of environmental factors, possibilities of their epidemiological surveillance, and control. Much emphasis is laid on ecological aspects of these diseases (haematophagous vectors and their life history; vertebrate hosts of zoonoses; habitats of the agents and their geographic distribution; natural focality of diseases). Particular zoonoses and sapronoses are then characterized in the following brief paragraphs: source of human infection; animal disease; transmission mode; human disease; epidemiology; diagnostics; therapy; geographic distribution.

Book excerpt: Originally published 1979 The Scientific Revolution in Victorian Medicine looks at the discovery of inhalation anaesthesia in 1846, and how it began a new era in surgery. The book looks at James Young Simpson’s demonstration of the value of chloroform as an anaesthetic, and how many surgeons quickly adopted it. The book also looks at the dangers of chloroform if mishandled and only after considerable controversy and numerous fatalities was its use thoroughly understood and established. Ten years later an even more lengthy struggle began over antiseptic surgery. The ‘germ’ theory, on which Lister’s technique was founded had few adherents among British surgeons, and his methods were deemed absurdly complicated. He was opposed and sometimes ridiculed by the most distinguished men in the profession, including Simpson. Over ten years were required to persuade the majority of British surgeons that Lister did actually achieve the results which he claimed and that it was possible for a competent surgeon to do equally well, if only he would take the trouble. This book shows that a great many factors interacted in delaying the introduction of these new ideas. The almost wholly unscientific nature of British medical education and practice before 1860 or 1870, detailed in the first chapter, was one factor; rivalry and distrust between London and Scotland was another. Genuine disadvantages in the new methods were not unimportant either, while personal animosities failure to face the facts, and fear of the unknowable consequences of change all played a significant part.
