Aussi, pouvoir initier un traitement orthodontique au moment propice constitue un avantage certain pour le patient, comme pour le praticien. Chaque période de la morphogenèse des arcades peut constituer une opportunité pour initier une thérapeutique orthodontique, chacune de ces périodes comportant des avantages et des inconvénients plus ou moins spécifiques. Il s'agira bien évidemment, pour le praticien, de choisir la période présentant un maximum d'avantages et un minimum d'inconvénients, pour le cas particulier du patient qui lui fait face. La décision d'entreprendre ou de différer un traitement ne pourra se faire qu'après une étude approfondie du cas, car il faut bien être conscient que nous avons à traiter des patients qui sont des cas particuliers et qu'il n'y a pas de recette. Cette décision devra résulter de la synthèse d'un ensemble de paramètres, à la fois orthodontiques (nature de l'anomalie, sévérité de l'anomalie, évolution probable en l'absence d'intervention immédiate, étiologie etc...) et non orthodontiques (motivation et coopération espérées, répercussions psychosociales éventuelles etc...) dont la conjonction permettra à l'orthodontiste de définir le moment le plus propice à son intervention. Malheureusement, dans un certain nombre de cas, il n'est pas donné au spécialiste la possibilité de choisir le moment le plus favorable pour initier sa thérapeutique, que ce soit à cause du dépistage tardif de la malocclusion ou, pire, d'une croyance erronée que l'on peut attendre.
