Book excerpt: A unique set of problems has been identified in joining osseointegrated implants and natural teeth with a fixed partial denture. Because of the relative mobility of the tooth with its periodontal ligament (PDL) attachment, it has been suggested that under masticatory loads, physiologic movement of the teeth may cause the prosthesis to act as a cantilever, generating increased load on the implant. The preservation of osseous integration depends in part on proper stress distribution within the bone and health of the marginal soft tissues. It would appear that an implant design in which the stress distribution most closely resembles that of natural teeth would be advantageous. Therefore analysis of the stresses generated to the bone via natural teeth with their PDL attachment and implants is desirable. The purpose of this study was to compare the distribution and magnitude of stress patterns generated in photoelastic resin transmitted by two different natural teeth and three different implant designs, with and without a simulated PDL. The natural teeth studied were a mandibular premolar and molar; the implants were Nobelpharma, Screwvent and Integral. All three implants are cylindrical but differ in their surface characteristics. Under a standard forty pound vertical and lateral load, the stress patterns were photographed in the field of a circular polariscope. The relative distribution and concentration of stresses in the PDL or peri-implant areas were analyzed and compared between the different teeth and implant designs. The load conditions of the models demonstrated no significant differences in the stress concentrations between the implant types. However, the configuration and amount of periodontal root support and the presence of a PDL were instrumental in causing a broader stress distribution within the periodontal and peri-implant areas.
