Book excerpt: Antimicrobial resistance (AMR) has been recognized as one of the greatest global threats to humans, animals, plants and ecosystems health threatening the achievement of the Sustainable Development Goals. In our globally connected world, resistance to antimicrobials may spread and circulate among humans, animals, plants and the environment, necessitating a “One Health” approach. While the One Health approach is relevant to all efforts to prevent and control AMR, this priority research agenda focuses on research areas at the interface between sectors. This research agenda is a joint product of the Quadripartite organizations –FAO, UNEP, WHO and WOAH - and a result of extensive stakeholder and expert engagement. A structured mixed-methods approach was used including reviews of academic and grey literature, online open global survey, and consensus exercise by modified Delphi method in which global experts prioritized research areas for the five pillars: transmission, integrated surveillance, interventions, behavioral insights and change, and economics and policy. We hope this research agenda will serve as a guiding tool for countries, research institutes and funding bodies to support for One Health AMR research, helping policymakers, researchers and the multidisciplinary scientific community to work together across sectors on solutions that will prevent and mitigate AMR on a national, regional and global scale as further evidence on research strategies, interventions and policies is required to understand what works, in which contexts and for whom.

Zoonoses—infectious diseases, such as SARS and mad cow, that originate in animals and spread to humans—reveal how intimately animal and human health are linked. Complicating this relationship further, when livestock are given antibiotics to increase growth, it can lead to resistant bacteria. Unfortunately, there are few formal channels for practitioners of human medicine and veterinary medicine to communicate about threats to public health. To address this problem, Dr. Laura H. Kahn and her colleagues are promoting the One Health concept, which seeks to increase communication and collaboration between professionals in human, animal, and environmental health. Despite its global spread, Africa, specifically sub-Saharan Africa, is the most affected by this malaise. Poor living conditions and inadequate access to sanitation and potable water supplies are among contributing factors that have influenced a high disease burden on the continent, requiring extensive antimicrobials. Weak health systems and the absence of firm policies further aggravate the problem, as the use of antimicrobials is mostly unregulated. The increasing demand for animal protein to meet the starving populations’ demands has also influenced the use of these antimicrobials, including those banned on other continents, for food animal production. The ripple effect of indiscriminate use in humans and animals is the massive discharge of antimicrobials, their residues, antimicrobial-resistant microorganisms and their associated genes into the environment. This 14-chapter unique masterpiece presents the AMR problem in African, addressing the various compartments of the One Health – humans, animals, and the environment, to illustrate the need for concerted efforts in the fight against AMR, especially in Africa. Authors from the four cardinal points present diverse aspects of AMR in Africa, starting with behavioural and social drivers of AMR in Africa. Antimicrobial stewardship in an African context is also discussed. AMR in humans is presented through studies on antibiotic-resistant neonates and nontyphoidal Salmonella infections and the clinical relevance of the genetics of viral resistance. Topics on AMR in mastitis, biosecurity in animal farming and the linkage between disinfectants and AMR are discussed. The environmental dimension of AMR is discussed, notably in the aquatic environment, and its implication for aquaculture and irrigation and using nanomaterials to treat polluted waters from such environments are highlighted. Finally, Africa’s rich floral diversity is portrayed as an eco-friendly and cost-effective approach to combat AMR. Hopefully, the work presented will spur greater collaboration between scientists, environmental, animal and human health practitioners, the general population, and policymakers to assimilate and implement the One Health approach to combating AMR, rather than working in silos on their various sectors

Those consequences will multiply if collaborative global action is not taken to address the spread of resistance. Major drivers of antimicrobial resistance in humans have been accelerated by inappropriate antimicrobial prescribing in health care practices; the inappropriate use of antimicrobials in livestock; and the promulgation of antibiotic resistance genes in the environment. To explore the issue of antimicrobial resistance, the Forum of Microbial Threats planned a public workshop. Participants explored issues of antimicrobial resistance through the lens of One Health, which is a collaborative approach of multiple disciplines - working locally, nationally, and globally - for strengthening systems to counter infectious diseases and related issues that threaten human, animal, and environmental health, with an end point of improving global health and achieving gains in development. They also discussed immediate and short-term actions and research needs that will have the greatest effect on reducing antimicrobial resistance, while taking into account the complexities of bridging different sectors and disciplines to address this global threat. This publication summarizes the presentations and discussions from the workshop.

With a dearth of new antibiotics coming to market, the need for action to avert a developing global crisis in health care is increasingly urgent. The World Health Organization has long recognized AMR as a growing global health threat, and the World Health Assembly, through several resolutions over two decades, has called upon member states and the international community to take measures to curtail the emergence and spread of AMR. The WHO Global strategy for the containment of antimicrobial resistance, published in 2001, set out a comprehensive set of recommendations for AMR control which remain valid today. This book examines the experiences with implementing some of those recommendations ten years on, the lessons learnt along the way and the remaining gaps.
