This problem is characterized by the insufficient availability of experts with knowledge of medical specialties involved, making it difficult to both the diagnosis process and the selection of appropriate treatment. In addition, in the treatment of this problem, there are situations of indetermination, uncertainty, vagueness, and incomplete data, hence the need to create novel computational techniques for supporting clinical decision making. This paper aims at creating a new model based on Neutrosophic Cognitive Maps that integrates diagnosis, treatment, and prediction processes for supporting decision making during the treatment of pregnancies with cardiovascular disease. In order to validate the model, a data base with 1019 cases provided by Cardiovascular and Pregnancy National Service / Gynecology and Obstetrics Hospital Ramón Gonzales Coros, was used. Finally, it is demonstrated in this work the efficient use of the proposed model for treating pregnant with cardiovascular diseases.

As the paradigm of healthcare delivery shifts from the current focus on the treatment and cure of acute illness to the needs of individuals with chronic illness, the system has to be redesigned to shift the focus to the more effective delivery of preventive care. The prevention and treatment of chronic illness is a multidisciplinary approach that relies heavily on decision support systems along with the incorporation of clinical pathways designed to provide cues for providers for the right intervention at the right time. Clinical decision support systems have been shown to improve the care of chronic illness by providing reminders for best practices and guideline driven pathways. Incorporating the clinical reminders into the daily workflow provides the highest levels of acceptance translating to more satisfied providers and improved clinical outcomes for patients. This project is about the construction of a clinical decision support system that would be integrated with the current electronic medical record (TouchWorks [trademark symbol] from AllScripts) in a multi-center, multi-specialty medical group in the Sacramento area. This thesis centers around the integration of multiple data sources to harvest knowledge about patients and the development of an engine that would provide the prompts in a just-in-time fashion at the decision making point of care. The knowledge from the various sources would be combined to provide disease specific alerts that would aid in the preventive measures delivered at the point of care. The connection from the decision support engine to the electronic medical record would be accomplished leveraging the Clinical Document Architecture (CDA) and the mandates of the Office of the National Coordinator for Health Information Technology (ONC-HIT) for "Meaningful Use" to provide the information into the inbox or the task list of the bedside clinician. The pilot project involves a small group of physicians at one primary care site and one common chronic condition namely congestive heart failure. Based on the lessons learned from this endeavor the effort will be expanded to diabetes, chronic lung disease and chronic kidney disease. Effective clinical decision support involves more than just the provision of reminders within the context of an electronic health system. It includes the integration of these reminders into the clinical workflow at the point-of-care such that these recommendations can be adapted into the care delivery. For this reason this thesis also explores the issues of knowledge maintenance and governance as the guidelines are revised and updated by the national specialty organizations. As such we explore the development and implementation of a clinical decision support system for the ongoing treatment of chronic illness. This is performed in a commercial EMR system in a private practice multispecialty group over multiple sites.

Book excerpt: In this work, we identify four main criteria and fifteen sub-criteria based on age, health status, a woman’s status, and the kind of job. The main and sub-criteria will be evaluated using a neutrosophic Analytic Hierarchy Process (AHP). Then, the COVID-19 vaccine alternatives will be ranked using a neutrosophic TOPSIS method. All the results obtained indicate that the healthcare personnel, people with high-risk health, elderly people, essential workers, pregnant and lactating mothers are the most prioritized people to take the vaccine dose first. Also, the results indicate that the most appropriate vaccine for patients and health workers have priority over other alternative vaccines.
