Book excerpt: Excerpt from A Manual of Pathology A desire has been expressed that this posthumous edition of the late Professor Coats's standard work should contain some brief indication of what he did, and what he was, in a life-time full of activity, but too short according to our human estimate for the tasks allotted to it. No more pathetic instance of frustrated hopes found than in. The simple statement that Joseph Coats, working life for nearly a quarter of a century towards a professorship of pathology, should have attained that official status barely five years before his death, and should have inaugurated a new pathological institute, mainly due to his own initiative and enthusiastic endeavours, on October 14th, 1896, to be used for only three seasons under'his direction, amid periods of ill-health, which ended in a fatal result on January 24th, 1899. The life that was thus closed, however, was one of remarkably sustained effort, and of absolute consistency of aim, from first to last. For more than twenty years before he became a professor in the University, Dr. Coats was the mainstay of scientific pathological teach ing in Glasgow. All his attainments, and all his resources, Were devoted consistently, and with marked success, to this one object; and the present volume, as well as the completed pathological institute which is now carried on by his successor, form the more or less permanent memorials of a life-time of arduous labour, carried on among the students of Glasgow University, by one who was as much respected by them as he was by those who later on became his colleagues in an official sense. As the oldest of these, a friend of Dr. Coats from his studentship onwards, and at all times working in co operation with him, I have been asked to write these few lines as a preface to his book. A more detailed notice of the man will be found in the British Medical Journal for 4th February, 1899. Book excerpt: Excerpt from A Manual of General Pathology: For the Use of Students and Practitioners of Medicine After perusing the fifth edition of Wagner's Pathology in 1872 I was strongly impressed with the idea that it was well worthy of reproduction in this country. N 0 book in the English language gives such a thorough résumé of the elements of medicine, and in none is the matter so arranged as to be available for both the student and the practitioner. This, the sixth German edition, is much improved, many references to publications in 1872 having been inserted; in many chapters the work is fully up to date. This elaboration is perhaps most evident in the sections on the theories of fever; where knowledge which could only be obtained by laborious work in a large city library is condensed within the compass of a few pages. I have permitted myself to make very few changes from the original; two or three pages from the sections on soil and climate, and some short paragraphs in the chapter on fever, have been omitted. A few foot-notes and bibliographical references (chiefly American), and an index of authors, have been added. My friend Dr. Van Duyn has translated at least two-thirds of the book. The proofreading and arrangement of the work have fallen to my share. In our translation we have encountered some difficulties, and those who have done the same kind of work will know how to make allowance for a num ber of awkward sentences in the English. We have endeavored to be literal and at the same time clear; and elegance of style has not been aimed at. In man, in the acute disease, and always in very severe cases, there may be suppression of the renal secretion, the exact causation of which is not known. Occasionally, in man, fatty degeneration of the renal cortex follows diphtheria. In the rabbit and guinea-pig this does not occur, but it occurs, in some cases, in the cat. In the rabbit the diphtheria poison may produce fatty de generation of the liver; in the guinea-pig this does not occur, but one of the characteristic actions of the bacillus in the guinea-pig is the production of double pleural effusion. Book excerpt: Excerpt from Manual Pathology and Practice, Being the Outline of the Course of Lectures Delivered The science of Pathology of necessity presupposes and is founded upon an acquaintance with the doctrines of Physiology. As Physiology is the history of the several organs of the animal body and their functions, in a natural and healthy state; so Pathology consists in a knowledge of the morbid conditions of the organs and their functions. Disease has been variously defined; it always implies some irregularity or aberration in the performance of one or more of the functions of the body. This irregularity of action may be owing to obvious alterations in the structure of some parts of the body, or it may occur without our being able to detect any change in the structure of any part. Hence the distinction of diseases into Functional and Organic - and hence the importance of a close inspection of the condition of the parts of bodies dead from disease - a description of which constitutes the useful and interesting department of morbid anatomy. Disease may be local or general. It is local when the cause producing it acts with special force upon some single part or organ, while yet no participation in the morbid consequences is extended to other parts. It is general when the primary local affection has been communicated or extended throughout the system. Each of these may produce the other. It is plausibly argued that all diseases are primarily local, and afterwards radiated or rendered universal by nervous sympathies, and by vitiation of the fluids of the body. General disease may also produce local affections, as in hepatic and splenic inflammation from the concussions of fever, and in gout and scrofula. Causes of disease divided in the books into Remote and Proximate. I prefer to use the terms Efficient and Constituent. The proximate cause has been absurdly enough regarded as the disease itself. An apology is due the many, to whom, during the last ten years, a new edition has been repeatedly promised. The writers only excuse for the failure to fulfill these promises has been the pressure of other work that has prevented such fulfillment. In the final accomplishment of these promises the book has been practically rewritten and more than doubled in size. The autopsy method given in the main text is a composite one, made up from the Rokitansky, Virchow, Chiari and Nauwerck methods, according to the judgment of the writer as to what was the best in these, and put together with modifications and additions arising out of his own experience. The aim has been to offer a method by which an autopsy can be performed with the greatest speed and ease, and at the same time with the greatest completeness, the various steps of the operation following in logical order in such a way that nothing can be lost or destroyed, and thereby revealing a complete picture of the pathologic conditions present. A choice of methods is offered whenever the aims of the examination may be so varied as to make variations in method advantageous. The general order of the autopsy is the same as that given in the Protocol Blank-book, the present book being designed as a guide and reference-book for that. The points to be noted in the examination of each region are given in connection with the method of examination of that region, and represent the condensed special pathology of the latter. This should be of great service to the beginner in autopsy work, as affording a concise but complete guide to the most important conditions of each region. A text-book on Special Pathology should be used as a reference book in connection with these condensed statements of special pathology. The technical methods for microscopic examination given in Part II have been brought up to date, and all recent methods of value included. Original methods have been given in preference to modifications; the latter, when of value, are also mentioned. As a rule that method has been chosen which in the light of the writers laboratory experience has yielded uniform the best results.
