Book excerpt: Colorectal cancer continues to rank as one the leading causes of cancer death in the United States on an annual basis. The majority of deaths are preventable through routine screening, beginning at the age of 50, as outlined and recommended in widely published clinical practice guidelines. The purpose of this research endeavor was to qualitatively examine how individuals over the age 50 perceive colorectal cancer and its causes, the individual, social, systemic, and environmental level obstacles faced when considering and attempting colorectal cancer screening as well as the communication strategies they employed or deemed appropriate toward utilizing screening options. Using in-depth one-on-one interviews, data was collected from 20 individuals 50 years or older living in the greater metropolitan Atlanta, GA area. Participants, a number of whom have medical background, reported knowledge of some known and widely reported risk factors for colorectal cancer. Family history and dietary habits were the most reported risk factors, followed by some mention of alcohol and tobacco consumption. Participants also communicated an awareness and knowledge of colonoscopy and fecal occult blood test (FOBTs) as colorectal cancer screening options. Regarding communication with healthcare providers, participants emphasized patient accountability in preparing for their encounter with their personal healthcare providers, particularly about colorectal cancer screening. Participants engaging in screening were driven to do so by a perceived positive physician-patient interaction where the patient feels respected, feels the physician acts as an advocate, and where the physician engages the patient in informed and shared decision making about screening options. Finally, participants' intention to act as defined by adherence to screening recommendation is further driven by the desire to avoid the experiences of family and friends who were diagnosed at a late stage with colorectal cancer. The findings from this study have implications for healthcare providers and researchers developing and implementing health interventions targeting men and women eligible for colorectal cancer screening.

Book excerpt: University of Washington Abstract Barriers to and Facilitators of Colorectal Cancer screening in Arab Americans: A Qualitative Study Muhammad Alsayid Chair of Supervisory Committee: Christian Dimaano, PhD, MPH Clinical Assistant Professor, Department of Health Services Purpose: To explore the barriers to and facilitators of colorectal cancer screening in Arab Americans as assessed through a qualitative study. Study Design: A qualitative study utilizing grounded theory conducted among focus groups from the Arab American community. Methods: We conducted two focus groups, where the first group had six participants and the second one had three participants. All participants were Muslims and males. The majority of participants had primary care physicians and underwent colorectal cancer screening. We used eight open-ended questions and added probes to collect more details. The focus groups' responses were audio-recorded and the recording was translated verbatim into English and transcribed. We utilized the Grounded Theory to build a conceptual model and two frameworks. A qualitative analysis was performed to evaluate the participants' responses. The data was reviewed and coded based on relevant words, sentences, and phrases. The codes were combined and themes were generated and placed into the categories of either barriers or facilitators. Results: Themes that emerged out of the qualitative data were placed into two categories: barriers and facilitators. Themes demonstrating barriers to colorectal cancer screening included disbelief in modern medicine, concerns about the procedure, and lack of communication with the physician. Three themes identified as facilitators to colorectal cancer screening included compliance and priority of health, access to healthcare, and awareness. Recommendation: Our recommendations include increasing the awareness of CRC and its consequences and explaining the health benefits of screening among the Arab American community. We also recommend providing cultural-based training to physicians in order to improve their communication skills with Arab American patients. Physicians should offer Arab American patients different CRCS methods to achieve higher levels of compliance.

Book excerpt: This paper explores the perspectives of Latino focus group participants regarding colorectal cancer (CRC) screening and gender. In the United States (US), the Latino community screens for CRC much less than other groups. Latino men can screen at low rates while their CRC mortality rates are high. The present focus group participants corroborated findings in which Latino study participants perceive threats to masculinity to be a barrier for men in CRC screening. Using Grounded Theory to analyze the focus group transcripts, a theory emerges that explores the potential role that Latino men's female partners can have to encourage their screening. There exists evidence that Latino men can benefit from their partners' support to complete their CRC screening. In theory, a woman's encouragement is successful in this context because she can mitigate the perceived threats to masculinity. Further research should test the strategy of female partner support for Latino men's CRC screening.
