Consequently, they have become increasingly consolidated into multi-user, small to midsized research facilities, located at many sites around the country. The proliferation of these facilities, however, has drawn calls for a careful assessment of best principles for their operation. With support from the Department of Energy and the National Science Foundation, the NRC carried out a study to characterize and discuss ways to optimize investments in materials research facility infrastructure with attention to midsize facilities. This report provides an assessment of the nature and importance of mid-sized facilities, their capabilities, challenges they face, current investment, and optimizing their effectiveness.

Developing guidelines presents a number of challenges including lack of transparent methodological practices, difficulty reconciling conflicting guidelines, and conflicts of interest. Clinical Practice Guidelines We Can Trust explores questions surrounding the quality of CPG development processes and the establishment of standards. It proposes eight standards for developing trustworthy clinical practice guidelines emphasizing transparency; management of conflict of interest ; systematic review-guideline development intersection; establishing evidence foundations for and rating strength of guideline recommendations; articulation of recommendations; external review; and updating. Clinical Practice Guidelines We Can Trust shows how clinical practice guidelines can enhance clinician and patient decision-making by translating complex scientific research findings into recommendations for clinical practice that are relevant to the individual patient encounter, instead of implementing a one size fits all approach to patient care. Book excerpt: Improvised explosive devices (IEDs) are a type of unconventional explosive weapon that can be deployed in a variety of ways, and can cause loss of life, injury, and property damage in both military and civilian environments. Terrorists, violent extremists, and criminals often choose IEDs because the ingredients, components, and instructions required to make IEDs are highly accessible. In many cases, precursor chemicals enable this criminal use of IEDs because they are used in the manufacture of homemade explosives (HMEs), which are often used as a component of IEDs. Many precursor chemicals are frequently used in industrial manufacturing and may be available as commercial products for personal use. Guides for making HMEs and instructions for constructing IEDs are widely available and can be easily found on the internet. Other countries restrict access to precursor chemicals in an effort to reduce the opportunity for HMEs to be used in IEDs. Although IED attacks have been less frequent in the United States than in other countries, IEDs remain a persistent domestic threat. Restricting access to precursor chemicals might contribute to reducing the threat of IED attacks and in turn prevent potentially devastating bombings, save lives, and reduce financial impacts. Reducing the Threat of Improvised Explosive Device Attacks by Restricting Access to Explosive Precursor Chemicals prioritizes precursor chemicals that can be used to make HMEs and analyzes the movement of those chemicals through United States commercial supply chains and identifies potential vulnerabilities. This report examines current United States and international regulation of the chemicals, and compares the economic, security, and other tradeoffs among potential control strategies.

Book excerpt: The Department of Toxic Substances Control (DTSC) of the State of California Environmental Protection Agency is in the process of complying with the Regulatory Structure Update. The Regulatory Structure Update is a comprehensive review and refocusing of California's system for identifying and regulating management of hazardous wastes. As part of this effort, the DTSC proposes to change its current waste classification system that categorizes wastes as hazardous or nonhazardous based on their toxicity. Under the proposed system there would be two risk-based thresholds rather than the single toxicity threshold currently used to distinguish between the wastes. Wastes that contain specific chemicals at concentrations that exceed the upper threshold will be designated as hazardous; those below the lower threshold will be nonhazardous; and those with chemical concentrations between the two thresholds will be "special" wastes and subject to variances for management and disposal. The proposed DTSC system combines toxicity information with short or long-term exposure information to determine the risks associated with the chemicals. Under section 57004 of the California Health and Safety Code, the scientific basis of the proposed waste classification system is subject to external scientific peer review by the National Academy of Sciences, the University of California, or other similar institution of higher learning or group of scientists. This report addresses that regulatory requirement.

It was the first systematic attempt at placing cultural and contextual factors as an integral component of the diagnostic process. The year was 1994 and its coming was ripe since the multicultural explosion due to migration, refugees, and globalization on the ethnic composition of the U.S. population made it compelling to strive for culturally attuned psychiatric care. Understanding the limitations of a dry symptomatological approach in helping clinicians grasp the intricacies of the experience, presentation, and course of mental illness, the NIMH Group on Culture and Diagnosis proposed to appraise, in close collaboration with the patient, the cultural framework of the patient's identity, illness experience, contextual factors, and clinician-patient relationship, and to narrate this along the lines of five major domains. By articulating the patient's experience and the standard symptomatological description of a case, the clinician may be better able to arrive at a more useful understanding of the case for clinical care purposes. Furthermore, attending to the context of the illness and the person of the patient may additionally enhance understanding of the case and enrich the database from which effective treatment can be planned. This reader is a rich collection of chapters relevant to the DSM-IV Cultural Formulation that covers the Cultural Formulation's historical and conceptual background, development, and characteristics.
