Understanding the risk factors for homelessness among Veterans and how these risk factors compare to the general population is important in developing identification and prevention programs for Veterans at risk for homelessness. This report was requested by VA Central Office and The National Center for Homelessness Among Veterans as part of that effort to identify what is known and what is not known about the prevalence of homelessness among Veterans, and about the risk factors for homelessness among Veterans, including risk factors related to military service and incarceration. The key questions were: Key Questions #1a. What is the prevalence and incidence of homelessness among Veterans? #1b. How has the prevalence and incidence of homelessness among Veterans changed over time? #1c. How prevalent are psychiatric illness, substance abuse, and chronic medical illness among homeless Veterans? Key Questions #2a. Which risk factors are associated with new homelessness or a return to homelessness among Veterans? How do these risk factors differ from non-Veteran populations? #2b. Have risk factors for homelessness among Veterans changed over time? Key Question #3. Are there factors specific to military service that increase the risk of homelessness, or is the increased risk a marker for pre-military comorbidities and social support deficiencies? The problem has elicited a variety of societal and public policy responses over the years, concomitant with fluctuations in the economy and changes in the demographics of and attitudes toward poor and disenfranchised citizens. In recent decades, federal agencies, nonprofit organizations, and the philanthropic community have worked hard to develop and implement programs to solve the challenges of homelessness, and progress has been made. However, much more remains to be done. Importantly, the results of various efforts, and especially the efforts to reduce homelessness among veterans in recent years, have shown that the problem of homelessness can be successfully addressed. Although a number of programs have been developed to meet the needs of persons experiencing homelessness, this report focuses on one particular type of intervention: permanent supportive housing (PSH). Permanent Supportive Housing focuses on the impact of PSH on health care outcomes and its cost-effectiveness. The report also addresses policy and program barriers that affect the ability to bring the PSH and other housing models to scale to address housing and health care needs.

Book excerpt: The primary focus of this research project is to explore what is known and not known about the prevalence of veteran homelessness and how various risk factors influence the likelihood of a veteran becoming homeless. This project also examines the effectiveness of interventions designed to address this social problem. This thesis is guided by two key research questions: 1) what factors are correlated with or related to homelessness among veterans?" and 2) what effect does Housing First, in comparison to VASH, transitional housing, and recovery based approaches, have on veteran homelessness?" The methodology that was utilized for this project consisted of a systematic literature search for scholarly peer-reviewed journal articles using several research databases and search engines such as EBSCOhost, Wiley Online Library, PubMed, Springer Link, NCBI, ProQuest, PsychiatryOnline, and Google Scholar. The results revealed that substance abuse, mental illness, and theater of war all significantly affect veteran homelessness. Findings also suggest that the most effective interventions for addressing the problem include housing subsidies, the Housing First Model, housing retention and transitional housing programs. More research on combating the social issue of veteran homelessness is needed, particularly among female veterans as most study samples are predominantly male.

Shortly after troops started returning from their deployments, some active-duty service members and veterans began experiencing mental health problems. Given the stressors associated with war, it is not surprising that some service members developed such mental health conditions as posttraumatic stress disorder, depression, and substance use disorder. Subsequent epidemiologic studies conducted on military and veteran populations that served in the operations in Afghanistan and Iraq provided scientific evidence that those who fought were in fact being diagnosed with mental illnesses and experiencing mental healthâ€"related outcomesâ€"in particular, suicideâ€"at a higher rate than the general population. This report provides a comprehensive assessment of the quality, capacity, and access to mental health care services for veterans who served in the Armed Forces in Operation Enduring Freedom/Operation Iraqi Freedom/Operation New Dawn. It includes an analysis of not only the quality and capacity of mental health care services within the Department of Veterans Affairs, but also barriers faced by patients in utilizing those services.

Book excerpt: This study is a systematic review to examine homeless veterans identified to be most at risk of unsuccessfully completing the VA’s housing program (HUD-VASH), which promotes the use of Housing First (HF) as it’s model for treating homelessness. The literature review identified those who were rural and experiencing comorbid substance use disorders (SUD) and mental health issues to likely be those who were most at risk. There were multiple reasons why this subgroup was most vulnerable including limited access to resources, higher levels of substance use and more serious mental health diagnoses, and chronic health needs. Both the literature review and systematic review in this study indicate a lack of evidence supporting Housing First especially its long-term effectiveness in the HUD-VASH program. The lack of evidence is due in part to the question of whether or not the programs examined in the literature are true HF models, which utilize all the core principles of HF. Further, the literature regarding HF predominantly measures the length of time to acquire permanent housing with little to no regard to harm reduction or rehabilitation of SUD and mental health needs. Further research is also needed to examine HUD-VASH effectiveness in treating rural veterans with comorbid SUD and mental health illnesses.

Book excerpt: As of December 2012, Operation Enduring Freedom (OEF) in Afghanistan and Operation Iraqi Freedom (OIF) in Iraq have resulted in the deployment of about 2.2 million troops; there have been 2,222 US fatalities in OEF and Operation New Dawn (OND)1 and 4,422 in OIF. The numbers of wounded US troops exceed 16,000 in Afghanistan and 32,000 in Iraq. In addition to deaths and morbidity, the operations have unforeseen consequences that are yet to be fully understood. In contrast with previous conflicts, the all-volunteer military has experienced numerous deployments of individual service members; has seen increased deployments of women, parents of young children, and reserve and National Guard troops; and in some cases has been subject to longer deployments and shorter times at home between deployments. Numerous reports in the popular press have made the public aware of issues that have pointed to the difficulty of military personnel in readjusting after returning from Iraq and Afghanistan. Many of those who have served in OEF and OIF readjust with few difficulties, but others have problems in readjusting to home, reconnecting with family members, finding employment, and returning to school. In response to the return of large numbers of veterans from Iraq and Afghanistan with physical-health and mental-health problems and to the growing readjustment needs of active duty service members, veterans, and their family members, Congress included Section 1661 of the National Defense Authorization Act for fiscal year 2008. That section required the secretary of defense, in consultation with the secretary of veterans affairs, to enter into an agreement with the National Academies for a study of the physical-health, mental-health, and other readjustment needs of members and former members of the armed forces who were deployed in OIF or OEF, their families, and their communities as a result of such deployment. The study consisted of two phases. The Phase 1 task was to conduct a preliminary assessment. The Phase 2 task was to provide a comprehensive assessment of the physical, psychologic, social, and economic effects of deployment on and identification of gaps in care for members and former members, their families, and their communities. The Phase 1 report was completed in March 2010 and delivered to the Department of Defense (DOD), the Department of Veterans Affairs (VA), and the relevant committees of the House of Representatives and the Senate. The secretaries of DOD and VA responded to the Phase 1 report in September 2010. Returning Home from Iraq and Afghanistan: Assessment of Readjustment Needs of Veterans, Service Members, and Their Families fulfills the requirement for Phase 2.

This project primarily examined the literature in order to assess the effectiveness of interventions shaped by the information technology systems data with respect to resource allocation and service provision for this social problem. This thesis was guided by this key research question: "Does the use of these systems enhance the efficiency of resource allocation and service effectiveness?" The methodology that was utilized for this project consisted of a systematic literature search for research articles that used data-driven solutions to address homelessness among veterans. Several research databases and search engines such as EBSCOhost, Wiley Online Library, PubMed, Springer Link, NCBI, ProQuest, PsychiatryOnline, and Google Scholar were used. Sixteen articles met the inclusion criteria. The results revealed that age, substance use disorders, and the U.S. Department of Housing and Urban Development-VA Supportive Housing all significantly affected veteran homelessness. Findings also suggest that the data captured and reviewed by the database management systems, such as Homeless Management Information System & the Homeless Operations Management and Evaluation System database, were all key components in producing viable and accurate descriptions of interventions that were producing quality results. More research on how technological interventions can boost the degree of quality, especially databases that are designed by social workers, is still needed.

Book excerpt: Although estimates of homelessness among Veterans have been declining since 2009 (the first year these data were reported), homelessness remains more common among Veterans compared to non-Veterans. Circumstances leading to homelessness are often complex because they can involve both community-level factors, such as local housing policies and market conditions, and factors at the individual level, such as having a mental health or substance use disorder. A social-ecological framework for understanding homelessness underscores that individual-level factors alone do not predict homelessness; rather, these factors characterize individuals who may be more vulnerable to broader societal and economic forces that create conditions of homelessness. Veterans may have unique individual-level vulnerabilities to homelessness, including those stemming from a history of combat exposure or experiences of military sexual trauma. They may also have unique protective factors, such as access to health care (including mental health care) and case management supports. The aim of this systematic review was to synthesize available evidence on factors associated with homelessness among Veterans to inform cross-VA efforts to reduce and prevent Veteran homelessness and identify research gaps. Ending Veteran homelessness is a VHA priority.
