By employing the method of conversation analysis, this book has probed into patient participation in Chinese community clinics. Declarative expressions, imperative expressions, and interrogative expressions are those frequently used by patients when they participate in consultations. Patients participate in all stages of communication: history taking, diagnosis, or treatment recommendations. There are four types of social actions performed by patients when they participate in the interaction: making requests, expressing concerns, showing alignment, or showing resistance to doctors. There exist two types of initiations of patient participation in the interaction: self-initiated patient participation and doctor-initiated patient participation. Self-initiated patient participation is closely related to patient agency, while doctor-initiated patient participation indicates the influence of doctors’ communicative behaviors on the involvement of patients.

Taking naturally occurring Chinese outpatient clinical conversations as the data and drawing from the analytic apparatuses provided by Conversation Analysis, this book represents an attempt to conduct a systematically descriptive and analytic study of the position and composition of treatment recommendations in doctor-patient interactions in Chinese outpatient clinics. By incorporating the analysis of epistemic and deontic orientations into the examination of particular linguistic resources employed to formulate the initiation, construction of, and responses to treatment recommendations, the study uncovers how Chinese doctors and patients steer the interactional trajectories of treatment decision-making through talk-in-interaction. The book can serve as a reference to the research on treatment decision-making in various kinds of Chinese medical settings. In addition, the insights from the study could provide a reference for understanding how Chinese doctors’ medical authority and patients’ agency are generally executed, challenged, and sustained via talk in the new era of patient-centered medical care.

Little research is available looking at WM being practiced in Asian countries, and misunderstanding about Eastern medicines such as TCM can result in unwarranted claims and suspicions. This volume contributes to research on doctor– patient communication by exploring the communication behaviors between doctors and older patients who are able to communicate independently in both TCM and WM practiced in mainland China and evaluating patient satisfaction with their medical experiences. The book reports findings and insights from three independent and methodologically diverse studies, drawing on data from 69 real- life medical consultations: 30 from TCM and 39 from WM. Using conversation analysis, the Roter Interaction Analysis System, and both quantitative and qualitative methods, Ying Jin examines the differences between TCM and WM to help reveal the dynamics of doctor– patient interactions, the contextual details, and the impact of the clinical culture on medical communication. Where many previous studies have focused on abortion in China as a state-mandated procedure to enforce the one-child policy, Lai looks at a new era, where abortion is primarily based on individuals’ decisions. While young women in China enjoy greater freedom to pursue their personal, sexual, and reproductive aspirations, their autonomy remains constrained by structural inequalities of gender, class, and migration status, which are reproduced through the intersection of state policies, market forces, and patriarchal family culture. In this book, Lai recounts the stories and presents the voices of unmarried young adult women, and documents the impact of sweeping socioeconomic transformation on their reproductive experiences in contemporary China amidst the ending of the one-child policy. Book excerpt: Guided by the methodology of conversation analysis (CA), this book explores how participants in Mandarin conversation display stance in the unfolding development of action and interaction, and, in particular, how this is accomplished through the use of two Mandarin final particles. Through a close examination of the sequential environments of these two particles and the interactional work accomplished by their use, the research presented in this book seeks to demonstrate how a participant-oriented, action-based micro approach to data can help us gain analytic leverage in understanding the functions and meanings of these particles – an area which has long posed a challenge to Chinese linguists. On the other hand, in utilizing a CA-based framework applied to Mandarin, this study also seeks to contribute to conversation analytic research by revealing previously uninvestigated language-specific phenomena while at the same time showing how talk-in-interaction in a non-western language, i.e., Mandarin, can also display the same striking systematicity and orderliness as observed in many western languages. As one of the pioneering CA studies of Mandarin, this book will be of interest to researchers in Chinese linguistics and conversation analysis, as well as those in fields which touch upon the relationships between languages and cultures.
