Book A Competitive Process for Procuring Health Services

Download or read book A Competitive Process for Procuring Health Services written by David Dranove and published by . This book was released on 2017 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Government agencies employ a variety of mechanisms for securing goods and services from the private sector. These include posting prices, reimbursing for costs, and soliciting competitive bids. Procurement of healthcare services offers several unique challenges. The supplier can influence the quantity of services provided. It is often difficult to even specify in advance exactly what services are to be purchased. Lastly, quality is difficult to measure. Healthcare purchasers have deployed a variety of payment mechanisms to cope with these challenges. We apply the theory of procurement to the case of cataract surgery. We recommend implementing a system that combines a gatekeeper with competitive bidding among operating physicians who must perform all necessary services, including treatment for complications, within a global fee. We conclude by discussing the strengths and limitations of this proposal.

Book excerpt: The choice of procurement scheme depends on a variety of factors, such as the presence of competing suppliers, the extent to which productive efficiency varies across providers, the degree of product differentiation, asymmetry of information between buyer and seller, and the importance of a fair and transparent procurement mechanism. [...] INTRODUCTION In Alberta, the body responsible for managing the provision of health care services to residents of the province is Alberta Health Services, which is also tasked with the challenge of ensuring access to high-quality care while keeping to a limited budget. [...] Subsequent procurement then relies on relational contracting, and the buyer and supplier end up negotiating over the terms of the contract and the division of the rents. [...] It Is Difficult to Specify the "Product" with Precision In a typical procurement situation, the purchaser specifies the characteristics of the product, and suppliers compete to sell the specified product. [...] The success of prospective payment clearly depends on the balance among • variation in hospital efficiency; • the ability of hospitals to increase efficiency in response to financial incentives; • whether there are substitutes in the production process not captured by the prospective payment; • whether it is costly to monitor and punish substitution done solely to increase profits; and • the abili.

Book Performance based Contracting for Health Services in Developing Countries

Download or read book Performance based Contracting for Health Services in Developing Countries written by Benjamin Loevinsohn and published by World Bank Publications. This book was released on 2008-01-01 with total page 226 pages. Available in PDF, EPUB and Kindle. Book excerpt: Despite the existence of effective interventions, there are many developing countries which are not on track to achieve the Millennium Development Goals (MDGs) for health. In many countries the delivery of health services is inadequate and one way of improving the situation is to contract with non-state providers. Contracting is a mechanism for a financing entity to procure a defined set of services from a non-state provider. Performance-based contracting is a type of contracting with: (a) a clear set of objectives and indicators; (b) systematic efforts to collect data to judge contractor performance; and (c) some consequences for the contractor, either rewards or sanctions, based on performance. Effective contracting for health services can be facilitated by using a systematic approach, described in this toolkit, that addresses key issues, including how to: 1. have a constructive dialogue with all stakeholders; 2. define the health services in terms of what services are to be delivered, where, the quantity of beneficiaries to be served, equity, and quality of care; 3. design the monitoring and evaluation to judge the performance of contractors; 4. select the contractors in a fair and transparent way; 5. arrange for effective contract management; 6. draft the contract and bidding documents; and 7. carry out the bidding process and successfully manage the contracts. The toolkit also includes a review of 14 evaluated examples of contracting in developing countries which concludes that the current weight of evidence indicates that contracting improves the coverage and quality of services rapidly. The six cases with controlled, before and after evaluations demonstrated large impact with themedian double difference (follow-up minus baseline in the experimental group minus follow-up minus baseline in the control) ranging from 9 to 26 percentage points.

Available in PDF, EPUB and Kindle. Book excerpt: "In its most recent market study report, the Competition Bureau published recommendations for federal, provincial and territorial governments to modernize public procurement processes used to acquire digital health care products and services. Improving health care through pro-competitive procurement policy is the second of three reports published as part of the Bureau's digital health care market study. The in-depth study examined how pro-competitive policies can foster innovation and bring about greater choice and access to digital health care services for Canadians. The latest report mainly focuses on public procurement processes which the provinces and territories use to acquire health care products and services to treat patients. In it, the Bureau identifies important barriers that prevent innovative suppliers from competing for public contracts. The report also makes major recommendations to governments across Canada to ensure that public procurement policy helps foster greater competition and innovation"--Provided by publisher.

Available in PDF, EPUB and Kindle. Book excerpt: Realizing the promise of digital technology will depend on the ability to share information across time and space from multiple devices, sources, systems, and organizations. The major barrier to progress is not technical; rather, it is in the failure of organizational demand and purchasing requirements. In contrast to many other industries, the purchasers of health care technologies have not marshaled their purchasing power to drive interoperability as a key requirement. Better procurement practices, supported by compatible interoperability platforms and architecture, will allow for better, safer patient care; reduced administrative workload for clinicians; protection from cybersecurity attacks; and significant financial savings across multiple markets. With funding support from the Gordon and Betty Moore Foundation, this National Academy of Medicine Special Publication represents a multi-stakeholder exploration of the path toward achieving large-scale interoperability through strategic acquisition of health information technology solutions and devices. In this publication, data exchanges over three environments are identified as critical to achieving interoperability: facility-to-facility (macro-tier); intra-facility (meso-tier); and at point-of-care (micro-tier). The publication further identifies the key characteristics of information exchange involved in health and health care, the nature of the requirements for functional interoperability in care processes, the mapping of those requirements into prevailing contracting practices, the specification of the steps necessary to achieve system-wide interoperability, and the proposal of a roadmap for using procurement specifications to engage those steps. The publication concludes with a series of checklists to be used by health care organizations and other stakeholders to accelerate progress in achieving system-wide interoperability.
