IN considering the conditions under which research in psychiatry is likely to be rewarded by success, we are at once met with the query, Have the conscious efforts of man or have accident and necessity heretofore been preponderant factors in fortuitous discoveries? Some philosophers contend that all conscious research is misdirected, wasteful, extravagant and unproductive expenditure of energy, that it is the whole duty of civilization to utilize the discoveries that accidently and spontaneously eventuate in the course of the ordinary affairs of life. There can be little doubt that most of the discoveries which gave man the power to rise from animalism to savagery, from savagery to barbarism and from barbarism to the beginning of civilization, were fortuitous accidents; but thereafter, on account of the complex field of science, and especially as it concerns us, of the complex field of pathology, advances have been made and they may be expected to result only on well-planned and conscious research by well organ, ized and well-trained squads of engineers in research. It is an undoubted fact that the earliest and most conspicuous advances of the human mind into the dark continent of ignorance and of the unknown have been wholly accidental and uncontemplated. Such accidental discoveries are still going on, but it is also a fact that during the last hundred years at least, conscious and purposeful researches have been undertaken and new and valuable discoveries, which never could be- disclosed by any conceivable accident, have been made with the aid of the systematic and orderly arrangement of facts previously discovered by necessity and accident alone. Constant and exact observation of the general health of the incurable insane, and a rigid investigation of the diseases under which they finally succumb, must be of high impor tance to us, in enabling us in numerous instances to detect those negations of curability, which ultimately interpose their veto upon all our hopes and promises. We have become famil iar with several physical complications, whose detection even at the very outset of insanity, or before mental aberration has yet been evinced, proclaim to us but too unmistakably, the utter hopelessness of the cases in which we observe them general paralysis, epilepsy, injuries of the brain, advanced organized lesions of the heart, the lungs, or of any other im portant organ, hardly fail to assure us of the incurability of their associated mental disease. It would seem but little to affect our prognosis whether the brain itself - that organ to which we are wont to ascribe immediate or sole mental sove reignty - is or is not directly implicated; its functional integ rity, whether primarily or only by reflex agency, impaired, will not be reconstructed so long as any lethal condition exists in any other important part of the system. Of all the morbid complications Of incurable insanity, none is perhaps more extensively present, or more certainly fatal, than pulmonary tubercular phthisis. On the contrary, certain sections of that field have been more or less arbitrarily selected, mainly on the ground that they yield fruitful results to psychological methods of investigation. I confidently anticipate that. In the future these methods will have a far wider application, but in the present state of our knowledge it must be frankly admitted that there are whole tracts of insanity in which they have only a very limited utility. This confession is all the more necessary on account of the tendency to unduly extensive generalisation evident in the work of many recent investigators. In View of the enormous complexity of mental processes, and the youthfulness ofpsychology, it is best to realise that progress must inevitably be slow, and that we must be content to feel our way to the scientific laws of the future.
