Methods: Three bacterial species were combined to create an in vitro biofilm. Treatment was completed with SN, SN/NaF, SDF, SDF1⁄2 or untreated (control). Results: The untreated group demonstrated significantly higher growth than all other treatment groups across the study. On the BHI-plates (1-day), there were significant differences between all treatments except SDF and SDF1⁄2. On the BHI-plates (3-days), SN/NaF was not significantly different from SDF or SDF1⁄2. On the L-MRS-plates (1-day), both SN treatment groups yielded significantly higher growth than the SDF groups. On the L-MRS-plates (3-days), SN yielded significantly higher growth than SN/NaF, SDF, and SDF1⁄2. Conclusion: SDF is more effective than SN/NaF, with the exception of BHI-plates (3-days) only and SN/NaF is more effective than SN on primarily S. mutans and L. acidophilus. There is evidence of a possible antimicrobial tolerance of oral bacteria to silver.

Book excerpt: Purpose: The use of silver-based antimicrobials is an emerging method for the treatment of dental caries. In this study, the authors compare the efficacy of the two most prominent silver- based therapeutics, silver diamine fluoride (SDF) and silver nitrate (AgNO3), on cariogenic and non-cariogenic multispecies biofilms. Currently there is a lack of studies comparing the efficacy of SDF to AgNO3. Methods: Plaque samples from anterior and posterior tooth sites from children presenting both with early childhood caries and caries-free children were collected, pooled, and utilized to create four ex vivo biofilm systems in artificial saliva. SDF and AgNO3 were administered to these biofilms and bacterial survival was quantified and compared to untreated controls. Results: Each of the four pooled sample types was applied to plates coated in artificial saliva + 1% sucrose. Both SDF and AgNO3 were very effective against plaque derived biofilms when compared to untreated biofilms (P0.05) in the potency of each compound. Conclusions: SDF and AgNO3 significantly inhibit ex vivo cariogenic and non-cariogenic biofilms at similar levels.

Book excerpt: Objectives: Silver diamine fluoride is approved by the United States Food and Drug Administration for intraoral human treatment of tooth hypersensitivity, and it has also been employed world-wide as an emerging method to arrest tooth decay. A 38% silver diamine fluoride formulation, comprised of 25% silver, 5% fluoride, and 8% ammonia as a solvent, is commercially available in the United States. One of the main mechanisms underlying the dental caries arrest potential of silver diamine fluoride is the silver component, which exerts pronounced antimicrobial activity against cariogenic bacteria. Interestingly, studies initiated in the late 1990s demonstrated marked susceptibility of periodontal bacterial pathogens to silver nitrate. However, efforts to develop silver-based, slow-release biodegradable wafers for subgingival placement into periodontal pockets were not commercially successful. At present, no commercial products are available which employ silver ions to combat periodontal bacterial pathogens in periodontal disease treatment. It is not known whether the 38% silver diamine fluoride product commercially available in the United States possesses antimicrobial activity against periodontal bacterial pathogens, and potentially, have application in periodontal therapeutic regimens. As a result, the objective of this study was to test the in vitro antimicrobial effects of silver diamine fluoride on freshly-isolated red and orange complex periodontal pathogens from severe human periodontitis lesions. Methods: Paper point subgingival biofilm samples from 24 adults with severe periodontitis that were to be discarded after microbiological analysis at the Temple University School of Dentistry Oral Microbiology Testing Service Laboratory were secondarily employed in this study. Dilution aliquots from each subgingival specimen were mixed with either 38% or 19% silver diamine fluoride, inoculated onto enriched Brucella blood agar plates, and incubated anaerobically for 7 days at 37°C. Bacterial species growing subsequent to the silver diamine fluoride exposure were considered to be resistant to that concentration of silver diamine fluoride. Total viable counts in silver diamine fluoride-exposed subgingival specimens were quantitated, and established phenotypic criteria employed to identify the following red and orange complex periodontal pathogens: Porphyromonas gingivalis, Tannerella forsythia, Prevotella intermedia/nigrescens, Parvimonas micra, Campylobacter rectus, Fusobacterium nucleatum group species, and Streptococcus constellatus. Other cultivable isolates recovered from silver diamine fluoride-exposed subgingival specimens were identified using matrix-assisted laser desorption-ionization time-of-flight (MALDI-TOF) mass spectrometry and Bruker MALDI Biotyper analytic software. Subgingival sample dilution aliquots not exposed to silver diamine fluoride were similarly processed as controls for comparison with silver diamine fluoride-exposed specimens Paired t-tests compared mean total subgingival viable counts, and mean total subgingival proportions of the evaluated anaerobic red and orange complex periodontal pathogens per patient, between subgingival biofilm samples exposed and not exposed in vitro to 38% or 19% silver diamine fluoride, with a P-value of

Abstract: The effect of silver diamine fluoride (SDF) on the mineral content of carious dentine and the preventive caries effect of erbium-doped yttrium aluminium garnet (Er: YAG) laser irradiation (2,940 nm) combined with SDF application under cariogenic biofilm challenge model was investigated. Firstly, the SDF mechanism of action in the remineralisation of demineralised dentine process observing changes in the mineral content in vitro was investigated. Secondly, we investigated the effect of various laser irradiations at sub-ablative energy levels on the fluoride uptake of 38% SDF treated dentine. Finally, we evaluated the effects of Er: YAG laser irradiation combined with SDF application on the fluoride uptake by dentine under a cariogenic biofilm challenge. Compared with a 10% sodium fluoride and potassium fluoride solution and a 42% silver nitrate solution with the same content of fluoride and silver respectively, 38% SDF is more effective in promoting remineralisation of demineralised dentine. The dentine samples treated with SDF did not show evident demineralised surface morphology. Although the lesion depth for SDF treated samples was not significantly deeper than for sodium fluoride treated samples, it was demonstrated that SDF has an inhibitory effect on dentine demineralization. The results of the SDF-laser experiment showed that carbon dioxide (CO2) laser irradiation (10,600 nm) followed by Er: YAG laser irradiation at specific sublablative levels promoted the higher fluoride uptake from SDF application than neodymium-doped yttrium aluminium garnet(Nd: YAG) laser (1,064 nm) and diode laser (810 nm) irradiation. This study provides guidelines for the selection of the appropriate laser irradiation parameters and types to promote an optimal level of fluoride uptake for dentine caries prevention. Er: YAG laser irradiation was elected among the laser types in the previous experiment for promoting higher levels of fluoride uptake and for its maximal absorption by water and high absorption by hydroxyapatite. The Er: YAG SDF-laser treated dentine was subjected cariogenic bacterial challenge to investigate the caries preventive effect of this treatment. The morphological surface of the treated samples presented many occluded dentinal tubules. There was a major increase in the hardness and elastic moduli values and the fluoride content also increased significantly after the remineralisation process. To conclude, this study demonstrated that SDF application inhibited dentine demineralization and arrested caries, and that SDF application followed by Er: YAG laser irradiation promoted an increase in fluoride uptake by sound dentine under a cariogenic biofilm challenge. Conventional dental treatments are either not available or are unaffordable in many rural areas. Innovative approaches are needed to improve this situation. Aim: This prospective clinical trial investigated the effectiveness of topical fluoride applications in arresting dentine caries. The first null hypothesis to be tested was that there were no differences in the effectiveness of applying a 38% silver diamine fluoride (SDF) solution (44,800 ppm F), a 5% sodium fluoride (NaF) varnish (22,600 ppm F) and water in arresting dentine caries. The second null hypothesis was that caries removal prior to fluoride application had no impact on its effectiveness in arresting dentine caries. Methods: A total of 375 Chinese kindergarten children, aged 3-5 years with carious upper anterior teeth were divided into five groups. Children in the first and second groups received annual applications of SDF. NaF was applied every three months onto the lesions of children in the third and fourth groups. For children in the first and third groups, soft carious tissues were removed with hand instruments prior to fluoride application at the baseline examination. The fifth group of children received annual applications of water. Follow-up examinations were performed every six months. The children's parents completed a questionnaire to assess parental satisfaction with the appearance of the upper anterior teeth of their child before treatment and again at 24 months after treatment. Results: A total of 308 children were followed for 30 months. Book excerpt: Objective: To investigate the efficacy of silver diamine fluoride (SDF) with 5% fluoride varnish (FV) in treating cavitated caries in young children and to explore the association between SDF treatment and oral health-related quality of life (OHRQoL). Methods: Children with active dentinal caries lesions (ICDAS 5 or 6) in primary teeth underwent treatment with 38% SDF and 5% FV at baseline and 4 months later. Treated lesions were assessed 4 and 8 months after baseline to determine arrest. Participants were deemed Completely Successful (CS) if all treated lesions were arrested and Incompletely Successful (IS) if at least one lesion was not arrested. The early childhood oral health impact scale (ECOHIS) questionnaire was completed at second and third visits. Statistical analyses included descriptive and bivariate methods. A P value ≤ 0.05 was significant. Results: Forty children with 239 caries lesions were enrolled; mean age 40.2 +/- 14.9 months and 45% male. Lesion arrest rates after one and two applications of SDF were 74.1% and 96.2%, respectively. Children who reportedly brushed twice daily were more likely to be in the CS group compared to those who brushed less (p=0.006). Participants in the CS group had a significantly lower mean baseline dmft score than those in the IS group (p= 0.048). No significant difference in OHRQoL was observed between CS and IS groups. Conclusions: SDF with 5% FV is an effective approach to the management of ECC; more than one application is recommended along with regular follow-up of patients and twice daily brushing. OHRQoL was not found to be affected by the level of success of SDF treatment.

That is why the microbicides' chemico-physical properties, their toxicity, ecotoxicity, effectiveness, and effective spectrum are described in greater detail. As mentioned before, the characteristics of microbicides play an important role. They have to be suited to the intended application to avoid detrimental effects on the properties and the quality of the material to be protected; also production processes in which microbicides are used to avoid disturbances by microbial action must not be disturbed by the presence of those microbicides.
