The principal aim was to compare the rate of alignment ofthe upper and lower labial segments using conventional Omni, active In- Ovation R and passive Damon 3MX self-ligating brackets. The second part of this study was to investigate the rate of extraction space closure in the buccal segments for all subjects. The third and final element of this study was to investigate the efficiency of treatment for all three bracket types. 100 eligible subjects undergoing upper and lower fixed appliance treatment with the loss of four premolars were randomly allocated to one of three treatment groups: conventional (Omni) n=20, active (In-Ovation R) n=38 or passive (Damon 3MX) n=42 self-ligating brackets. Study models were taken for all subjects at the start and at twelve week intervals until completion of treatment. The same archwire sequence was used in each case. The labial segment alignment of each study model was measured using a digital Vernier calliper and Little's Index of Irregularity. Once alignment was achieved, space closure was carried out using 150g NiTi coil springs and once again measured using a digital calliper. Analysis of data using linear mixed models demonstrated a significant effect of bracket type on the rate of alignment in the mandible (p=0.026), but not the maxilla (p=0.277). In the mandible, rate of alignment was significantly greater with conventional brackets (Omni). There was no significant difference between the two self-ligating brackets. The rate of space closure was significantly faster in males when compared with females for both the In-Ovation R (p=0.001) and the Omni brackets (p=0.003). This was also found to be the case for Damon 3MX brackets although this was not shown to be statistically significant. The results demonstrated a significant difference between the conventional and the two self- ligating brackets in relation to the total chair side treatment time, with the Omni bracket subjects total treatment time taking on average 50 minutes longer. They also showed a significant difference in the total number of appointments between the In-Ovation R and the Omni bracket (p=0.0009), with the In-Ovation R requiring fewer appointments. In all other analyses there were no significant differences. II.

Book excerpt: Marketing strategies for self-ligating brackets have focused on less need for extractions and faster treatment. Recent meta-analyses have not found these claims to be well supported, but they point out limitations of the current evidence. These meta-analyses also conclude that additional, well-designed studies are indicated to improve our evidence base. The purpose of this retrospective cohort study is to assess the effects and efficiency of treatment with self-ligating brackets compared to conventional brackets. A secondary purpose is to identify any pretreatment malocclusion characteristics associated with choice of either self-ligating or conventional brackets. Patients treated in the private offices of two practitioners who simultaneously used both self-ligating and conventional brackets were selected for this study. The self-ligating subjects were selected first, and for each subject, an age and gender matched subject was chosen from the respective office. The self-ligation subjects were consecutively treated adolescents and young adults (11-25 years-old) who completed comprehensive fixed appliance therapy between January 1, 2011 and April 31, 2012. Patients with craniofacial anomalies, surgical treatment, treatment with arch expansion using expanders, interdisciplinary treatment, and incomplete records were excluded. The outcome measures were changes in the intercanine width, intermolar width, and arch length for both arches, change in mandibular incisor inclination, final PAR score, percent PAR reduction, overall treatment time, total number of visits, number of emergencies, number of broken brackets, and other emergencies such as a wire sliding to one side or the loss of ligation. All measurements were performed on digital models or digital lateral cephalograms in a blinded manner. PAR scores were measured by two calibrated assessors and the average of the two scores were used. Data were analyzed using the independent sample t-test, chi-square or fisher exact test and linear regression analysis. The final sample comprised 74 patients from Clinician 1, and 34 patients from Clinician 2. The two clinicians exhibited significant differences in treatment duration, as well as in treatment strategies, such as the use of the SureSmile technique. Therefore, the decision was made not to pool the data from the two offices. For Clinician 1, no significant differences were observed between self-ligating and conventional brackets for the outcomes described above, other than increased arch length in both arches in the self-ligating group. The self-ligation patients treated by Clinician 2 demonstrated a significant increase in transverse dimension, less percent reduction in PAR score, less treatment time, fewer visits, and more wire sliding emergencies compared to the conventional bracket group. While some significant findings were observed, the small sample and lack of consistent findings between the two clinicians makes it difficult to draw strong conclusions.

Book excerpt: This study evaluates the anterior-posterior dental control of orthodontic patients treated with the Damon2 passive self-ligating bracket system. These patients were compared to patients treated with a conventional twin .022 (MBT prescription) bracket system. The Damon2 group (n=30) and the MBT group (n=33) consisted of patients who presented with mild to moderate dental crowding (measured by irregularity index). All patients had full complements of teeth, first molar to first molar, in both maxillary and mandibular arches. They were skeletal and dental Class I and all were orthodontically treated non-extraction. The measurements collected were U1-SN angle, Interincisal angle, IMPA and SN-GoGn. Measurements were taken from pre and post treatment lateral cephalometric head films. Measurements were repeated and paired samples t-tests were used to compare consistency of repeated measures. The t-test was used to compare the treatment change for the above measurements and also to compare the post-treatment results for the measurements. The IMPA treatment changes was also tested for high and low MPA patients in the two treatment groups. Data indicates that there was no significant difference between the two bracket systems with regard to treatment changes for incisor position. A comparison of the post-treatment measurements also shows no significant difference. Finally, no difference was found between the bracket systems for treatment of high and low MPA patients.
