Book excerpt: Excerpt from A Clinical Treatise on Diseases of the Breast I have often been struck with the difficulties in diagnosis and treatment of maladies of the breast, experienced even by surgeons of acknowledged eminence and repute. I have from time to time collected notes of many cases of interest in mammary disease from the practice of the Cambridge Hospital, and from the various other public institutions with which I have been connected. A careful study has been made of the Registrar's notes at St. George's Hospital between 1865 and 1895, a period of thirty years. In like manner, I have availed myself of published reports of the various London Hospitals, and I have studied the specimens of mammary disease in all the London Museums, and at the Royal College of Surgeons. I have also bestowed a more cursory examina tion upon the specimens in the Leeds and Edinburgh Museums. From these materials and my own experience the present book is compiled. It contains the results of the experience of a man whose opportunities for observation have been immense, and whose diligence in the use of them is familiar to all professional readers. It can fall to the lot of but few to have seen so much, and it is not in the power of many to have observed so faithfully. Whatever advances, therefore, may be made in theoretical medical and surgical knowledge, the facts upon which this work is based can never lose their value. It represents the views entertained by a large class of the older surgeons, who, while they have not, on the one hand, remained insensible to the value of modern microscopical researches, have, on the other, hesitated to accord entire belief to doctrines which tend to overturn, on what they consider insufficient grounds, the experience derived from years of observation. Cooper has great confidence, In these cases, in a mixture of borax one dram, alcohol halfan ounce, and water three ounces. For my own part, in case Of failure of the above remedies, the cucumber ointment, the poplar Ointment, l or simple cerate, I have found nothing better than a weak solution of the nitrate of silver or sulphate of zinc. The ointment of the white precipitate, to be soon mentioned, is also an excellent remedy. But it must always be kept In mind that the suction of the child Is the determining cause of the evil, and that the greater part Of these remedies may give rise to accidents, if there be any remains of them upon the nipple when it is seized by the child. The best remedy therefore In these cases is a well applied artificial nipple. With the assistance Of this latter, simple attention to cleanliness or any one Of the local applications above mentioned, we rarely fail to effect a cure in a few days. Thus, lotions of brandy as preventive, Of Goulard's lotion, of Oil and lime water with styptic solutions, the mild oint ments or the white precipitate as remedies, and the use Of a well made artificial nipple, are the resources, sanctioned by experience, against the tumefaction and excoriations of the nipple. The poplar Ointment is a sedative application composed Of poplar buds, poppy leaves, and belladonna as the principal ingredients. (p.
