Book excerpt: The effects of LIPUS on orthodontic tooth movement (OTM) has been studied before, but there is a big controversy over the role of LIPUS on the rate of OTM. Also the mechanism of this process is largely unknown. The aim of the present study was to investigate the effect of different intensities of LIPUS on OTM, as well as anabolic and catabolic properties on the bone. For this reason 24 animals were randomly divided into four groups, and on each group different intensities of LIPUS were applied (control, 15 mW/cm2, 30 mW/cm2, and 50 mW/cm2 groups). All animals received orthodontic appliance and tooth movement on the right side of the maxilla, and left side was used as control. A combination of micro-CT analysis, TRAP staining and immunohistochemistry was used to analyze and compare the rate of tooth movement, bone density, osteoclastic activity and bone markers between the four groups. Our results showed the rate of tooth movement changes depending on the intensity of ultrasound. There was a significant increase in the rate of tooth movement in 30 mW/cm2 group compared to 15 and 50 mW/cm2. Osteoclasts number was increased in 30 group compared to 50 and 15 mW/cm2, and also expression of important paracrine factors detected, such as VEGF and PGE2, varied based on the intensity. In conclusion OTM changes with using LIPUS. These changes in OTM happen through series of biological reactions controlling PGE2 and VEGF expression as well as osteoclast numbers in response to the force. LIPUS has an application in clinical orthodontics for acceleration or retention purposes.

Injectable gels in endodontics is a new theme in dentistry that bioengineering skills can advance its development, specifically by producing clinically safe and effective gels with regeneration and antibacterial properties. Engineered products often need to be tested in vivo before being clinical in dentistry; thus, one chapter is dedicated to reviewing applicable animal models in dental research. Book excerpt: Orthodontically induced root resorption (OIIRR) is an undesirable consequence of orthodontic treatment and may lead to permanent loss of root structure. A previous clinical trial demonstrated the effect of LIPUS on tipping type of OTM and reported that LIPUS can promote cementogenesis. However, the effect of LIPUS on other types of OTM still remains unclear. Therefore, the aim of this study was to evaluate the effects of LIPUS on OIIRR caused by torque in human subjects. The study sample consisted of 10 healthy human patients between the ages of 12-35 years at the start of the study who required extraction of all first premolars as a part of their routine orthodontic treatment plan. A 15 degree twist was applied in the arch wire using 0.019 x 0.025 TMA wire producing a torque (buccal root) at the bracket level of approximately 5 N-mm. Using a split mouth design, randomization and blinding, one side of the arch received LIPUS for twenty minutes per day for four weeks at an incident intensity of 30mW/cm2 of the transducers surface area. The other side served as a self-control which received a sham transducer. After 4 weeks, all first premolars were extracted for regular orthodontic treatment and a micro-CT analysis was performed of these extracted teeth. Comparison between the two groups was performed using Linear mixed model analysis. LIPUS treated teeth showed significantly less damage compared to control as LIPUS reduced the total volume of resorption lacunae by a mean difference of (0.537 +/- 0.092 mm3) (P

Book excerpt: Background: Supplemental vibration has been introduced to orthodontics to accelerate the rate of tooth movement and to reduce patient discomfort. Purpose: This study investigated the impact of the AcceleDent® device on Invisalign® treatment. Specifically, we wished to determine if using the AcceleDent® device allowed aligners to be changed with a one-week, rather than the usual two-week, regimen. Design: This 2-armed, randomized, triple-blinded, active-controlled clinical trial was carried out in 2 orthodontic practices in Seattle, WA and Vancouver, BC with a 1:1 allocation ratio. 26 adult subjects (12 male and 14 female, mean age=33) were randomly allocated to either an active or control AcceleDent® device. The control devices were exactly like the active devices, but the coupler that transmitted the force to the mouthpiece was removed. All patients were placed on a one-week regimen for changing aligners and were evaluated by their orthodontist every three weeks. If the fit was adequate, the patients would continue with treatment. If there was a lack of fit, based on pre-established guidelines, the patient would be considered a non-completer. The primary outcome compared the percentage of completers in the active and control groups. A secondary outcome measured the final incisor irregularity and the change in incisor irregularity for those who completed their regimen of aligners. Results: Fisher’s exact test showed no significant difference in completion rates between the two groups (Active = 77% completion, Control = 85% completion, p=.99). Independent-sample t-tests showed no significant difference between final incisor irregularity (p=0.75) or the change in incisor irregularity (p=0.74) between the two groups. Compliance with aligners and the AcceleDent® device were similar in both groups. Conclusions: This study found no evidence that AcceleDent® impacts the ability to complete a series of aligners with a one-week change regimen or the final alignment achieved in this adult patient. The purpose of this prospective clinical study was to evaluate the efficacy of tooth movement with Invisalign. Methods. 401 anterior teeth were measured from the virtual models of 37 participants treated with Anterior Invisalign. The virtual model of the predicted-tooth position was superimposed over the virtual model of the achieved-tooth position. Results. The mean accuracy of tooth movement with Invisalign was 41%. The most accurate movement was lingual constriction (47.1%), and the least accurate movement was extrusion (29.6%). The accuracy of canine rotation was significantly lower than the rotation of all other teeth, with the exception of the upper lateral. Lingual crown tip was significantly more accurate than labial crown tip, particularly for the upper central and lateral incisors. Conclusion. We still have much to learn regarding the biomechanics and efficacy of the Invisalign system.
