Book excerpt: The mechanisms underlying acute and chronic lung diseases are complex, reflecting the interplay between multiple cell types, their microenvironment and exogenous challenges. While traditional in vitro cell culture approaches have been instrumental in advancing our knowledge of cellular signalling and function, they typically lack the cell-cell and cell-matrix interactions that define the niche in which lung cell and tissue functions emerge. Implementing in vivo and ex vivo three-dimensional (3D) models more realistically mimicking the in vivo cell-extracellular matrix (ECM) crosstalk should facilitate a considerable leap towards better understanding lung diseases and thus in investigating new pharmacological tools. We are experiencing a revolution in our understanding of the cell types that deposit and remodel ECM in the lung, the dynamic spatial composition of the ECM and cell-cell interactions during disease, and the influence of ECM and cell-derived cues on lung cell biology. In concert, the ex vivo and in vitro models that are being used to examine the role of the 3D microenvironment of the cell in the lung are rapidly developing. The European Respiratory Society has partnered with Frontiers in Pharmacology to launch this research topic in conjunction with the ERS Research Seminar “Innovative 3D models for understanding mechanisms underlying lung diseases: powerful tools for translational research”.

Globally, more than 30% of the diseases that cause disability and death are due to organ fibrosis. However, there have been so far only a few effective treatments for organ fibrosis. Idiopathic pulmonary fibrosis (IPF) is progressive, lethal fibrotic lung disease. Approximately, 10/100,000 people globally suffer from pulmonary fibrosis, and the incidence is on the rise. The median survival for patients is only 3-5 years. Currently there is no effective treatment for lung fibrosis. Therefore, new insights into lung fibrosis and therapeutic strategy should be explored and developed. This Research Topic aims to deepen the understanding and research on the pathogenesis of lung fibrosis to develop effective treatments. Studies have shown that effective lung repair and regeneration are critical biological processes for the restoration of original physiological function of lung after injury. The intrinsic ability of regeneration and repair is often suppressed in lung with chronic injury or the diseased organs, leading to fibrosis and loss of function. Targeting key pathways in the process of lung regeneration can promote the repair of the impaired lung and alleviate fibrosis in animal models. How to induce regeneration and repair of the impaired organ to alleviate fibrosis is a frontier research topic. Hence, this Research Topic focuses on the cellular and molecular mechanisms of lung regeneration, repair, and fibrosis. Moreover, this topic will also focus on the study of lung development to provide new insights into the pathogenesis of lung injury and fibrosis.

Book excerpt: The focus of this study was to investigate the histology of tissue formed when fetal (16-20 days gestation) and neonatal (2 days old) rat lung cells were grown in a collagen-glycosaminoglycan scaffold. This project employed a collagen-GAG scaffold specifically developed for tissue engineering and investigated the effect of this substratum on the formation of lung histotypic structures in vitro. A cell isolation procedure was developed whereby 19-days gestation type II alveolar cells reaggregated to form alveolar-like structures. The effects of selected scaffold design variables including pore diameter and degradation rate of the substratum on lung tissue regeneration were explored. Lung cell behavior revealed as the cells interact with an analog of the extracellular matrix was also examined. Differences in fetal and neonatal lung cell behavior were identified using histological analysis. Lung cells were obtained from Sprague-Dawley rats after 16-, 19-, and 20-days of gestation and at 2 days after term. These cells were seeded into type I collagen-GAG matrices, sized 8mm in diameter by 2mm in thickness. The medium used, F12K and Ham's nutrient mixture, was supplemented with 10% fetal bovine serum. A seeding density between 1 to 5 million cells per sponge sample was used. Histology studies were performed at termination periods of 2, 14, and 28 days. This paper describes the in vitro formation and long-term maintenance of alveolar-like structures from enzymatically dissociated 19-days gestation fetal rat lung cells cultured on a collagen sponge substrate as a model system for lung tissue engineering.
